乙肝表面抗體與年齡的相關(guān)研究及影響因素

【摘要】:目的:本文通過(guò)調(diào)查分析發(fā)現(xiàn),乙肝表面抗體隨著年齡的增長(zhǎng)有一定的降低趨勢(shì),這種趨勢(shì)是否由年齡的原因造成還是由其他的因素所導(dǎo)致,是本文研究的主要目的。 方法:實(shí)驗(yàn)對(duì)象為來(lái)自大連醫(yī)科大學(xué)附屬第一醫(yī)院的非肝炎患者,乙肝表面抗體陽(yáng)性(抗HBs含量10IU/L)、乙肝表面抗原陰性(HBsAg0.05IU/L)、e抗原陰性(HBeAg1IU/L),天門(mén)冬氨酸氨基轉(zhuǎn)移酶正常(男15AST40U/L,女15AST35U/L),丙氨酸氨基轉(zhuǎn)移酶正常(男9ALT50U/L,女7ALT40U/L),,選取110例,男55例,女55例。實(shí)驗(yàn)分組情況:根據(jù)年齡及e抗體,核心抗體陰性或陽(yáng)性分為四組:?jiǎn)慰贵w陽(yáng)性青年組:年齡為24-26歲,e抗體陰性,核心抗體陰性。男10例,女10例。單抗體陽(yáng)性中年組:年齡為45-50歲,e抗體陰性,核心抗體陰性。男10例,女10例!2和/或3抗體”陽(yáng)性青年組:年齡為24-26歲,e抗體或者核心抗體任意一個(gè)陽(yáng)性,或者二者均陽(yáng)性。男15例,女15例!2和/或3抗體”陽(yáng)性中年組:年齡為45-50歲,e抗體或者核心抗體任意一個(gè)陽(yáng)性,或者二者均陽(yáng)性。男20例,女20例。用羅氏cobas e601全自動(dòng)電化學(xué)發(fā)光免疫分析儀,雙抗體夾心法原理測(cè)定各組標(biāo)本乙肝表面抗體,按照一定比例稀釋樣本,直至最后一個(gè)稀釋度抗HBs含量10IU/L;用LIAISON化學(xué)發(fā)光免疫分析系統(tǒng),測(cè)定“2和/或3抗體”陽(yáng)性青年組和老年組標(biāo)本的巨細(xì)胞病毒IgG抗體(CMV-IgG),按照一定比例稀釋樣本,直至最后一個(gè)稀釋度抗CMV-IgG14AU/ml。比較分析四組間抗體活性、效價(jià)的變化。 結(jié)果:“2和/或3抗體”陽(yáng)性青年組和中年組表面抗體水平相比,青年組表面抗體活性水平高于老年組,有統(tǒng)計(jì)學(xué)顯著性差異(P<0.05)!2和/或3抗體”陽(yáng)性青年組和中年組巨細(xì)胞病毒IgG抗體活性相比,中年組巨細(xì)胞病毒IgG抗體活性高于青年組,有統(tǒng)計(jì)學(xué)顯著性差異(P<0.05)。青年組單抗體陽(yáng)性組和“2和/或3抗體”陽(yáng)組表面抗體活性相比,“2和/或3抗體”陽(yáng)性組表面抗體活性水平高于單抗體陽(yáng)組,有統(tǒng)計(jì)學(xué)顯著性差異(P<0.01);中年組單抗體陽(yáng)性組和“2和/或3抗體”陽(yáng)組表面抗體活性相比,“2和/或3抗體”陽(yáng)性組表面抗體活性水平高于單抗體陽(yáng)組,有統(tǒng)計(jì)學(xué)顯著性差異(P<0.01) 結(jié)論:乙肝表面抗體隨著年齡增長(zhǎng),呈一定的下降趨勢(shì)。其可能原因是與乙型肝炎病毒感染進(jìn)程中抗體反應(yīng)模式有所不同有關(guān)。
[Abstract]:Objective: through investigation and analysis, we found that hepatitis B surface antibody with the increase of age has a certain trend of decline, whether this trend is caused by age or other factors, is the main purpose of this study. Methods: the subjects were non-hepatitis patients from the first affiliated Hospital of Dalian Medical University. Hepatitis B surface antibody positive (10 HBs / L, HBsAg 0.05 IUU / L negative, HBeAg 1 IUP / L), aspartate aminotransferase normal (15AST 40U / L, female 15AST 35U / L P / L), alanine aminotransferase (9ALT50UL / L, female 7ALT40UL / L, male 55, male 55). 55 cases of female. The experimental group: according to age and e antibody, core antibody negative or positive was divided into four groups: single antibody positive youth group: age 24-26 years old, negative core antibody, male 10 cases, single antibody positive young group, age 24-26 years old, core antibody negative, male 10 cases, single antibody positive youth group: age 24-26 years old. Female 10. Single antibody positive middle-aged group: age 45-50 years of age negative, core antibody negative. Male 10, female 10. "2 and / or 3" positive youth group: age 24 to 26 years old, or core antibody any positive, Or both are positive. 15 males and 15 females. "2 and / or 3" positive middle-aged group: age 45-50 years of age, either one of them is positive for the antibody or core antibody, or both. 20 cases of female. Using cobas e601 automatic electrochemiluminescence immunoanalyzer, double antibody sandwich method was used to determine the surface antibody of hepatitis B in each group, and the samples were diluted according to a certain proportion. Using the LIAISON chemiluminescence immunoassay system, the cytomegalovirus IgG antibody against cytomegalovirus (CMV-IgG) was detected in the samples of the "2 and / or 3" positive youth group and the elderly group, and the samples were diluted according to a certain proportion. Up to the last dilution, the changes of antibody activity and titer among the four groups were compared and analyzed. Results: the level of surface antibody activity in the young group was higher than that in the elderly group, compared with that in the middle age group and the 2 and / or 3 antibody positive group, the level of surface antibody activity in the young group was higher than that in the old group. There was significant difference (P < 0.05). The activity of cytomegalovirus IgG antibody in middle age group was higher than that in young group, and the activity of IgG antibody in middle age group was higher than that in young group. There was significant difference (P < 0.05). Compared with "2 and / or 3" positive group, the surface antibody activity of "2 and / or 3" positive group was higher than that of single antibody positive group. There was a significant difference (P < 0.01), the surface antibody activity of "2 and / or 3 antibody" positive group was higher than that of single antibody positive group (P < 0.01), and the surface antibody activity of "2 and / or 3 antibody" positive group was higher than that of single antibody positive group (P < 0.01). Conclusion: hepatitis B surface antibody tends to decrease with age, which may be related to the different reaction pattern of antibody in the process of hepatitis B virus infection.
