【摘要】：目的探討烏司他丁對(duì)腸缺血再灌注損傷大鼠的保護(hù)作用及其機(jī)制。方法將60只健康Wistar大鼠隨機(jī)分為對(duì)照組(A組)、腸缺血再灌注后0 h組(B1組)、腸缺血再灌注后2 h組(B2組)、烏司他丁治療腸缺血再灌注后0 h組(C1)、烏司他丁治療腸缺血再灌注后2 h組(C2組)各12只。B1、B2、C1、C2組夾閉腸系膜上動(dòng)脈(SMA)60 min制作腸缺血再灌注損傷模型,并于手術(shù)前經(jīng)尾靜脈B1、B2組各注入生理鹽水2 m L,C1、C2組各注入烏司他丁5×104U/kg;A組僅分離SMA,不夾閉血管。取A、B1、B2、C1、C2各組大鼠的小腸組織,光鏡下觀察組織學(xué)改變,TUNEL法檢測(cè)小腸細(xì)胞凋亡率。結(jié)果光鏡觀察發(fā)現(xiàn),A組小腸黏膜基本正常;B1組小腸黏膜及絨毛排列輕度紊亂,無(wú)黏膜變性、壞死,出現(xiàn)上皮下間隙擴(kuò)大,腺體輕度受損及毛細(xì)血管充血;B2組小腸絨毛水腫明顯,黏膜及絨毛排列明顯紊亂,黏膜上皮脫落呈現(xiàn)糜爛,腺體嚴(yán)重受損;以上變化在絨毛頂端尤為顯著。C1、C2組小腸黏膜充血、水腫及上皮糜爛壞死的程度均較B1、B2組減輕。B1、B2組大鼠細(xì)胞凋亡率較A組升高(P均0.01)。C1組與B1組、C2組與B2組比較細(xì)胞凋亡率均降低(P均0.05)。結(jié)論烏司他丁可抑制小腸細(xì)胞凋亡,有助于減輕由于缺血再灌注引起的腸道損傷。
[Abstract]:Objective to investigate the protective effect of ulinastatin on intestinal ischemia-reperfusion injury in rats and its mechanism. Methods 60 healthy Wistar rats were randomly divided into control group (group A), intestinal ischemia reperfusion group (group B 1), intestinal ischemia reperfusion group (group B 1) and intestinal ischemia reperfusion group (group B 2). H group B 2 group, ulinastatin treatment group 0 h after intestinal ischemia reperfusion, and ulinastatin treatment group 2 h after intestinal ischemia reperfusion group (group C 2) 12 rats each group clamped superior mesenteric artery SMA-60 min to make intestinal ischemia-reperfusion injury model. Before operation, the small intestine tissues of rats in each group were injected with normal saline 2 mL / L C _ (1) C _ (2) and ulinastatin 5 脳 10 ~ (4) U / kg ~ (-1) 路kg ~ (-1) A, only SMAs were isolated and blood vessels were not clipped. The small intestine tissues of rats in each group were taken from AMB B _ (1) B _ (2) B _ (2) C _ (1) C _ (2) group. Results the intestinal mucosa in group A was basically normal and the villi in group B1 were slightly disarranged, no degeneration of mucosa, necrosis, and enlargement of epigastric space were observed by Tunel method under light microscope, and Tunel method was used to detect the apoptosis rate of small intestine in group B.Results in group A, the intestinal mucosa was basically normal, and the villi in group B1 were slightly disarranged. Small intestinal villi edema, mucosal and villus arrangement disorder, erosion of mucosal epithelium and serious damage of glands were observed in small intestine villi of group B2 with slight glandular damage and capillary hyperemia, and the above changes were especially significant at the top of villous villi. The degree of edema and epithelial erosion and necrosis were decreased compared with that of B1B _ 2 group. The apoptosis rate of B _ (1) B _ 2 group was higher than that of A group (P < 0.01). C _ 1 group and B _ 1 group C _ 2 group and B _ 2 group were lower than B _ 2 group. Conclusion Ulinastatin can inhibit small intestinal cell apoptosis. It helps to alleviate the intestinal injury caused by ischemia reperfusion.
