【摘要】：目的觀察瞳孔成形聯(lián)合虹膜夾人工晶狀體植入術(shù)治療外傷性無(wú)晶狀體眼的療效。方法回顧性分析在我院采用瞳孔成形聯(lián)合虹膜夾人工晶狀體植入術(shù)治療的外傷后無(wú)晶狀體眼患者17例17眼的臨床資料。觀察術(shù)后視力、角膜內(nèi)皮細(xì)胞計(jì)數(shù)、眼壓、人工晶狀體位置、瞳孔情況及術(shù)后并發(fā)癥。結(jié)果 17例患者均順利修補(bǔ)損傷虹膜,瞳孔成形后成功植入虹膜夾人工晶狀體,未發(fā)生嚴(yán)重手術(shù)并發(fā)癥,人工晶狀體位置穩(wěn)定。術(shù)后6個(gè)月裸眼視力(0.52±0.17),較術(shù)前視力(0.04±0.02)及術(shù)前最佳矯正視力(0.44±0.13)提高。術(shù)后6個(gè)月角膜內(nèi)皮細(xì)胞計(jì)數(shù)為(1993.88±127.24)個(gè)·mm~(-2),較術(shù)前(2178.88±132.61)個(gè)·mm~(-2)減少(P0.05),但術(shù)后未發(fā)現(xiàn)角膜失代償。術(shù)前眼壓(15.91±2.73)mm Hg(1 k Pa=7.5 mm Hg)與術(shù)后眼壓(16.69±2.61)mm Hg無(wú)明顯差異(P0.05)。術(shù)后瞳孔大小(4.4±0.2)mm較術(shù)前(5.5±0.3)mm縮小(P0.05),患者術(shù)后無(wú)明顯畏光、眩光、復(fù)視等不適癥狀。結(jié)論瞳孔成形聯(lián)合虹膜夾人工晶狀體植入術(shù)是治療外傷虹膜損傷并無(wú)有效晶狀體囊膜支持的無(wú)晶狀體眼的安全有效方法。
[Abstract]:Objective to observe the effect of pupil shaping combined with iris clip intraocular lens implantation in the treatment of traumatic aphakic eyes. Methods the clinical data of 17 cases (17 eyes) of post-traumatic aphakic eyes treated with pupil shaping combined with iris clip intraocular lens implantation in our hospital were analyzed retrospectively. the clinical data of 17 cases (17 eyes) of post-traumatic aphakic eyes were analyzed. Postoperative visual acuity, corneal endothelial cell count, intraocular pressure, intraocular lens position, pupil and postoperative complications were observed. Results all the 17 patients successfully repaired the injured iris. After pupil formation, the iris clip intraocular lens was successfully implanted. no serious surgical complications occurred and the position of intraocular lens was stable. 6 months after operation, the uncorrected visual acuity (0.52 鹵0.17) was higher than that before operation (0.04 鹵0.02) and the best corrected visual acuity (0.44 鹵0.13). The corneal endothelial cell count was (1993.88 鹵127.24) mm~ (- 2) at 6 months after operation, which was lower than that before operation (2178.88 鹵132.61) mm~ (- 2) (P 0.05), but no corneal decompensation was found after operation. There was no significant difference between preoperative intraocular pressure (15.91 鹵2.73) mm Hg (1 k Pa=7.5 mm Hg) and postoperative intraocular pressure (16.69 鹵2.61) mm Hg) (P 0.05). The pupil size after operation was (4.4 鹵0.2) mm smaller than that before operation (P 0.05). There were no obvious discomfort symptoms such as photophobia, glare and amblyopia after operation. Conclusion pupil shaping combined with iris clip intraocular lens implantation is a safe and effective method for the treatment of aphakic eyes with traumatic iris injury without effective lens capsule support.
