Tla聲門型喉癌經(jīng)激光手術(shù)與放射治療后嗓音質(zhì)量的系統(tǒng)綜述和Meta分析

【摘要】:目的:通過系統(tǒng)綜述和meta分析評估T1a聲門型喉癌經(jīng)激光手術(shù)和放射治療后的嗓音質(zhì)量。方法:通過計算機檢索PubMed、Web of Science、Embase數(shù)據(jù)庫、Cochrane、中國生物文獻數(shù)據(jù)庫(CBM)、相關(guān)期刊論文(CNKI)、萬方數(shù)字期刊全文數(shù)據(jù)庫等,從創(chuàng)建到2016年10月31日的文獻數(shù)據(jù),納入比較T1a聲門型喉癌經(jīng)激光手術(shù)和放射治療后的嗓音功能的隨機對照研究和隊列研究文獻,由兩位共同作者同時對搜索的相關(guān)文獻進行閱讀和篩選,從中提取與研究目的相關(guān)的數(shù)據(jù),然后對提取的數(shù)據(jù)用Review Manager 5.3軟件進行meta分析,分析和比較兩種治療方式后嗓音障礙指數(shù)(VHI)、最大發(fā)音時間(MPT),基頻(F0),基頻微擾(Jitter),振幅微擾(Shimmer),氣流量(AFR)。結(jié)果:通過文獻檢索和篩選結(jié)果共納入符合要求的文獻15篇,其中1篇隨機對照研究,1篇前瞻性研究,13篇回顧性研究。系統(tǒng)綜述和meta分析結(jié)果表明,激光手術(shù)組比放射治療組最大發(fā)音時間(MPT)短[MD=-2.26,95%CI(-3.94,-0.59),P=0.008],基頻(F0)高[MD=14.41,95%CI(10.14,18.19),P0.00001],兩種治療方式后嗓音障礙指數(shù)量表(VHI)[MD=5.86,95%CI(-5.22,16.94),P=0.30],基頻微擾(Jitter)[MD=0.75,95%CI(-0.28,1.79),P=0.15],振幅微擾(Shimmer)[MD=1.07,95%CI(-0.60,2.75,P=0.21]、氣流量(AFR)[MD=21.46,95%CI(-78.79,121.72),P=0.67]以及GRBAS評估差異均無明顯統(tǒng)計學(xué)意義。結(jié)論:在對T1a聲門型喉癌患者治療中,放射治療在MPT和F0方面稍優(yōu)于激光手術(shù),但嗓音障礙指數(shù)量表(VHI)、基頻微擾(Jitter)、振幅微擾(Shimmer)、氣流量(AFR)以及GRBAS評估差異均無明顯統(tǒng)計學(xué)意義。因此,迫切需要更多的多中心的隨機對照試驗來驗證這個結(jié)論。
[Abstract]:Objective: to evaluate the voice quality of T1a glottic carcinoma after laser surgery and radiotherapy by systematic review and meta analysis. Methods: the PubMed,Web of Science,Embase database was searched by computer, and the Cochrane, Chinese biological literature database (CBM), Chinese journal full-text database (CNKI), Wanfang digital journal full-text database was searched. From the establishment to October 31, 2016, the literature data were included in the randomized controlled study and cohort study to compare the voice function of T1a glottic carcinoma after laser surgery and radiotherapy. The related literature was read and screened by the two co-authors at the same time, and the data related to the purpose of the study were extracted, and then the extracted data were analyzed by meta with Review Manager 5.3 software. The (VHI), maximum pronunciation time (MPT), fundamental frequency (F0) and the fundamental frequency perturbation (Jitter), amplitude perturbation (Shimmer), air flow (AFR). After the two treatments were analyzed and compared. Results: a total of 15 literatures were included in the literature retrieval and screening results, including 1 randomized controlled study, 1 prospective study and 13 retrospective studies.
