【摘要】：目的：白內(nèi)障術(shù)前常規(guī)應(yīng)用沖洗液沖洗結(jié)膜囊預(yù)防眼內(nèi)炎的發(fā)生。如何正確評估與選擇合適的結(jié)膜囊沖洗液,是一個值得探討的問題。本文分析了白內(nèi)障術(shù)前應(yīng)用聚維酮碘、生理鹽水及抗生素沖洗結(jié)膜囊的臨床對照試驗(yàn),討論聚維酮碘較其他沖洗液沖洗結(jié)膜囊是否能有效降低結(jié)膜囊細(xì)菌陽性率,預(yù)防眼內(nèi)炎癥發(fā)生。 方法：檢索Medline、Pubmed、Ovid、The Cochrane Library、EMBASE及中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫發(fā)表的有關(guān)聚維酮碘、生理鹽水及抗生素在白內(nèi)障術(shù)前沖洗結(jié)膜囊效果的對照研究,并輔以手工檢索、因特網(wǎng)搜索。共納入4篇文章6項試驗(yàn),分為兩組對其療效進(jìn)行分析。 結(jié)果：聚維酮碘沖洗組與生理鹽水沖洗組相比能顯著降低結(jié)膜囊細(xì)菌陽性率,差異有顯著性(P0.0001),合并RR值為0.15,95%CI為[0.05,0.45],與抗生素沖洗組比較有顯著差別,差異有顯著性(P0.05),合并RR值為0.38,95%CI為[0.17,0.83]。 結(jié)論：白內(nèi)障術(shù)前應(yīng)用聚維酮碘沖洗結(jié)膜囊較生理鹽水及抗生素沖洗能顯著降低術(shù)后結(jié)膜囊細(xì)菌培養(yǎng)陽性率,預(yù)防眼內(nèi)炎癥的發(fā)生。
[Abstract]:Objective: to prevent the occurrence of endophthalmitis by washing conjunctival capsule with washing solution before cataract surgery. How to correctly evaluate and select the appropriate conjunctival capsule flushing solution is a problem worthy of discussion. This paper analyzes the clinical controlled trial of washing conjunctival capsule with povidone iodine, saline and antibiotics before cataract surgery, and discusses whether povidone iodine can effectively reduce the bacterial positive rate of conjunctival capsule compared with other flushing solution. Prevent the occurrence of inflammation in the eye. Methods: a comparative study of povidone iodine, saline and antibiotics published by Medline,Pubmed,Ovid,The Cochrane Library,EMBASE and Chinese Biomedical Literature Database on the effect of washing conjunctival capsule before cataract surgery was searched, supplemented by manual retrieval. Internet search. A total of 4 articles and 6 trials were included and divided into two groups to analyze the curative effect. Results: compared with saline washing group, povidone iodine washing group significantly decreased the positive rate of conjunctival capsule bacteria (P0.0001). The RR value of povidone iodine washing group was 0.15, 95% CI was 0.05, 0.45). Compared with antibiotic flushing group, there was significant difference (P 0.05). The combined RR value was 0.38 and 95% CI was 0.17, 0.83 respectively. Conclusion: compared with saline and antibiotic flushing, washing conjunctival capsule with povidone iodine before cataract surgery can significantly reduce the positive rate of bacterial culture in conjunctival capsule and prevent the occurrence of intraocular inflammation.
