中鼻甲泡與鼻中隔偏曲及兩者與慢性鼻—鼻竇炎關(guān)系

【摘要】:一、目的: 探討球型中鼻甲泡的發(fā)生與鼻中隔偏曲是否相關(guān),并分析兩者與慢性鼻-鼻竇炎的關(guān)系。 二、方法: 1.972例(1944側(cè))患者的鼻竇冠狀位CT片,為本研究各種統(tǒng)計(jì)數(shù)據(jù)的來源。 2.統(tǒng)計(jì)鼻中隔偏曲和無鼻中隔偏曲、高位偏曲和低位偏曲的球型鼻甲泡發(fā)生情況,并測量球型鼻甲泡橫徑,探討球型鼻甲泡發(fā)生與鼻中隔偏曲的關(guān)系。 3.觀察慢性鼻-鼻竇炎和球型鼻甲泡,并分析球型鼻甲泡與慢性鼻-鼻竇炎發(fā)生的關(guān)系。在CT片上測量球型鼻甲泡的橫徑數(shù)值,得出球型鼻甲泡的橫徑平均值,評估各球型鼻甲泡的大小。在同一CT片上,觀察單側(cè)和雙側(cè)有球型鼻甲泡者與慢性鼻-鼻竇炎的發(fā)生情況,并測量球型鼻甲泡橫徑,探討其大小與慢性鼻-鼻竇炎的關(guān)系。 4.觀察鼻中隔偏曲,把鼻中隔偏曲分為高位偏曲和低位偏曲,分析鼻中隔偏曲與慢性鼻-鼻竇炎發(fā)生的關(guān)系。 三、結(jié)果: 1.972例共1944側(cè)的慢性鼻-鼻竇炎發(fā)生率為22.63%(440側(cè)/1944側(cè))。球型鼻甲泡患者142例,發(fā)生率9.72%(189側(cè)/1944側(cè)),其中單側(cè)球型鼻甲泡95例,雙側(cè)球型鼻甲泡47例。鼻中隔偏曲發(fā)生率18.72%(182例/972例),其中高位偏曲60例,低位偏曲122例。 2.在本組資料中,182例鼻中隔偏曲組球型鼻甲泡共31例,其中1例為雙側(cè)球型鼻甲泡,30例為單側(cè)球型鼻甲泡,占8.79%(32側(cè)/364側(cè)),790例無鼻中隔偏曲組球型鼻甲泡共111例,其中48例為雙側(cè)球型鼻甲泡,63例為單側(cè)球型鼻甲泡,占10.06%(159側(cè)/1580側(cè)),兩組間球型鼻甲泡發(fā)生率無顯著性差異(P0.05)。高位偏曲患者60例,存在單側(cè)球型鼻甲泡20例,均位于鼻中隔凹側(cè),無雙側(cè)球型鼻甲泡,占16.67%(20側(cè)/120側(cè)),在低位偏曲患者122例中,出現(xiàn)單側(cè)球型鼻甲泡10例,位于鼻中隔凹側(cè),雙側(cè)球型鼻甲泡1例,占4.92%(20側(cè)/244側(cè)),高位偏曲組與低位偏曲組球型鼻甲泡發(fā)生率有顯著性差異(P0.05)。鼻中隔偏曲組和無鼻中隔偏曲組球型鼻甲泡橫徑均值分別為5.58±1.79mm和4.12±1.78mm,兩者之間差異具有顯著性(P0.01)。高位偏曲組和低位偏曲組球型鼻甲泡橫徑均值為5.92±1.87mm和4.80±1.35mm,兩組間無顯著性差異(P0.05)。 3.142例球型鼻甲泡患者中,95例單側(cè)球型鼻甲泡者有鼻甲泡側(cè)發(fā)生慢性鼻-鼻竇炎35側(cè),占36.84%(35側(cè)/95側(cè)),無鼻甲泡側(cè)發(fā)生慢性鼻-鼻竇炎20側(cè),占21.05%(20側(cè)/95側(cè)),兩組間慢性鼻-鼻竇炎發(fā)生率有顯著性差異(P0.05)。47例雙側(cè)球型鼻甲泡組慢性鼻-鼻竇炎共47側(cè),占50.00%(47側(cè)/94側(cè)),830例無球型鼻甲泡組慢性鼻-鼻竇炎共338側(cè),占20.36%(338側(cè)/1660側(cè)),兩組間慢性鼻-鼻竇炎發(fā)生率有顯著性差異(P0.01)。單側(cè)、雙側(cè)球型鼻甲泡不同鼻甲泡大小者慢性鼻-鼻竇炎發(fā)生率無顯著性差異(P0.05)。 4.182例鼻中隔偏曲組慢性鼻-鼻竇炎有60例,占32.97%(60例/182例),無鼻中隔偏曲患者790例,有慢性鼻-鼻竇炎217例,占27.47%(217例/790例),兩組間慢性鼻-鼻竇炎發(fā)生率無顯著性差異(P0.05)。60例高位偏曲患組,發(fā)生慢性鼻-鼻竇炎27例,占45.00%(27例/60例),122例低位偏曲患中,發(fā)生慢性鼻-鼻竇炎33例,發(fā)生率27.05%(33例/122例),兩組間慢性鼻-鼻竇炎發(fā)生率有顯著性差異(P0.05)。無論高位偏曲還是低位偏曲,鼻中隔凸側(cè)與凹側(cè)慢性鼻-鼻竇炎發(fā)生率無顯著性差異(P0.05)。 四、結(jié)論: 1.鼻中隔高位偏曲與球型鼻甲泡的發(fā)生有關(guān),尤其是凹側(cè)易出現(xiàn)球型鼻甲泡。 2.球型鼻甲泡與慢性鼻-鼻竇炎的發(fā)生有相關(guān)性,但慢性鼻-鼻竇炎的發(fā)生與球型鼻甲泡的大小無關(guān)。 3.鼻中隔高位偏曲與慢性鼻-鼻竇炎有相關(guān)性。
