【摘要】：目的研究腺樣體肥大(AH)患兒中胃內(nèi)容物鼻咽部反流與腺樣體免疫狀態(tài)的相關(guān)性。方法收集AH病例,分為AH不伴扁桃體肥大組(A組),AH伴扁桃體肥大組(B組),收集無腺樣體和扁桃體肥大的患兒作為對照組。采用酶聯(lián)免疫吸附(ELISA)法檢測鼻咽部分泌物中胃蛋白酶的表達,免疫組織化學法檢測腺樣體組織中BCL-2、PCNA、CD45Ro、CD20的表達,進一步分析胃蛋白酶水平與BCL-2、PCNA、CD45Ro、CD20的相關(guān)性。此外,分析上述指標表達水平與AH程度的相關(guān)性。結(jié)果 A組和B組中胃蛋白酶平均水平分別為(2.91±1.69)ng/mL,(2.70±1.42)ng/mL,胃蛋白酶陽性率分別為57.1%和54.6%,均高于對照組(P0.01),而A組和B組的胃蛋白酶平均水平和陽性率差異無統(tǒng)計學意義(P0.05)。BCL-2、PCNA、CD45Ro、CD20在腺樣體組織中的表達,B組有高于A組的趨勢,但僅CD20表達差異有統(tǒng)計學意義(P=0.042)。胃蛋白酶與BCL-2、PCNA、CD45Ro,CD20表達呈正相關(guān)(均P0.01),相關(guān)系數(shù)分別為0.64,0.66,0.63,0.58。此外,在根據(jù)AH程度分組的亞組比較中,胃蛋白酶陽性率差異有統(tǒng)計學意義(P=0.022),BCL-2、PCNA、CD45Ro、CD20的表達水平均與腺樣體的分度有關(guān)(P0.01)。結(jié)論鼻咽部分泌物中胃蛋白酶表達水平與腺樣體的免疫反應(yīng)活躍程度相關(guān),胃內(nèi)容物鼻咽部反流可能是AH的可能病因。
[Abstract]:Objective to study the correlation between nasopharyngeal reflux and adenoid immune status in children with adenoid hypertrophy (AH). Methods AH cases were divided into AH group without tonsillar hypertrophy group (group A), AH with tonsillar hypertrophy group). Children without adenoid and tonsil hypertrophy were collected as control group.
