細(xì)胞因子預(yù)測(cè)鼻咽癌患者放療敏感性及預(yù)后的相關(guān)性研究

【摘要】:目的:探討鼻咽癌患者放療前后細(xì)胞因子的表達(dá)與放療敏感性及預(yù)后的關(guān)系。 方法:采用流式細(xì)胞小球微陣列術(shù)(Cytometric Bead Array, CBA)檢測(cè)80例鼻咽癌患者放療前后外周血血清中細(xì)胞因子IFN-γ、TNF-α、IL-2、IL-4、IL-5、IL-10的表達(dá),并結(jié)合臨床資料進(jìn)行比較分析。 結(jié)果:IFN-γ和TNF-α的表達(dá)與鼻咽癌分期呈正相關(guān),分期越早,表達(dá)越低(P值分別為0.05和0.02)。治療前非角化型癌患者IFN-γ表達(dá)高于角化型癌患者(18.19vs.10.11,P=0.012)。放療后IL-5和IL-10的表達(dá)較放療前升高(4.25vs.3.50,P=0.000;3.02vs.2.58,P=0.020)。放療前IL-5高表達(dá)的患者比低表達(dá)的患者療效好,OR為0.19(95%CI:0.05_0.78,p=0.021),但與總生存無(wú)明顯相關(guān)性。TNF-α和IL-2表達(dá)與患者預(yù)后相關(guān),放療前TNF-α高表達(dá)的患者比低表達(dá)的患者增加了超過(guò)兩倍的死亡風(fēng)險(xiǎn)(HR=2.66,95%CI:1.04-6.78,p=0.041),相反,IL-2高表達(dá)的患者比低表達(dá)的患者有較低的死亡風(fēng)險(xiǎn)(HR=0.31,95%CI:0.13-0.75,p=0.009)。Kaplan-Meier曲線分析同樣顯示了放療前IL-2和TNF-α的不同表達(dá)組曲線差異具有統(tǒng)計(jì)學(xué)意義。 結(jié)論:放療前TNF-α的高表達(dá)、IL-2的低表達(dá)是鼻咽癌患者獨(dú)立的不良預(yù)后因子。
[Abstract]:Objective: to investigate the relationship between the expression of cytokines and radiotherapy sensitivity and prognosis in patients with nasopharyngeal carcinoma (NPC) before and after radiotherapy. Methods: the expression of cytokines IFN- 緯, TNF- 偽 and IL-2,IL-4,IL-5,IL-10 in peripheral blood of 80 patients with nasopharyngeal carcinoma before and after radiotherapy was detected by flow cytometric microarray (Cytometric Bead Array, CBA). The clinical data were compared and analyzed. Results: the expression of IFN- 緯 and TNF- 偽 was positively correlated with the staging of NPC, and the earlier the staging, the lower the expression (P = 0 05 and 0 02, respectively). Before treatment, the expression of IFN- 緯 in patients with non-keratinous carcinoma was higher than that in patients with keratogenic carcinoma (18.19 vs.10.11, P < 0.012). The expression of IL-5 and IL-10 after radiotherapy was higher than that before radiotherapy (4.25 vs. 3.50, 0.000 鹵3.52 vs 2.58, P = 0.020). The patients with high expression of IL-5 before radiotherapy had better curative effect than those with low expression, OR was 0.19 (95% CI = 0.05? 0.78, p = 0.021), but there was no significant correlation between the expression of TNF-偽 and IL-2 and the prognosis of the patients, but there was no significant correlation between the expression of TNF-偽 and the prognosis of the patients. Patients with high expression of TNF- 偽 before radiotherapy were more than twice as likely to die (HR=2.66,95%CI:1.04-6.78,p=0.041) as those with low expression. On the contrary, Patients with high expression of IL-2 have a lower risk of death (HR=0.31,95%CI:0.13-0.75,) than those with low expression. Kaplan-Meier curve analysis also showed that the curves of different expression groups of IL-2 and TNF- 偽 were significantly different before radiotherapy. Conclusion: the high expression of TNF- 偽 and the low expression of IL-2 before radiotherapy are independent adverse prognostic factors in NPC patients.
