頻域OCT檢測視野前青光眼黃斑區(qū)視網(wǎng)膜神經(jīng)節(jié)細胞復(fù)合體厚度的初步研究

【摘要】:目的探討黃斑區(qū)視網(wǎng)膜神經(jīng)節(jié)細胞復(fù)合體(GCC)厚度檢測對于診斷視野前青光眼(PPG)患者的意義。設(shè)計自身對照研究和病例對照研究。研究對象POAG患者20例40眼(無視野缺損眼為PPG組20眼,視野缺損眼為POAG組20眼)和正常對照者30例30眼。方法采用RTvue SD-OCT進行黃斑區(qū)GCC厚度和視盤周圍視網(wǎng)膜神經(jīng)纖維層(RNFL)厚度檢測,應(yīng)用受試者工作特征曲線下面積(AUC)評價各參數(shù)區(qū)分PPG與正常眼的能力。主要指標GCC與RNFL的厚度以及二者AUC值。結(jié)果 PPG組平均GCC厚度(Avg GCC)、上方GCC厚度(Sup GCC)、下方GCC厚度(Inf GCC)、平均RNFL厚度(Avg RNFL)、上方RNFL厚度(Sup RNFL)、下方RNFL厚度(Inf RNFL)與正常對照組相比均變薄,差異有統(tǒng)計學意義(t=5.508,5.402,5.576,6.517,5.849,5.392,P均0.001),但均厚于POAG組,差異有統(tǒng)計學意義(t=4.778,3.698,4.593,5.209,3.757,5.450,P均≤0.002)。PPG組GCC整體丟失體積(GCC-GLV)和局部丟失體積(GCC-FLV)均較正常對照組增大,差異有統(tǒng)計學意義(t=6.539,2.515,P0.001,P=0.021),但較POAG組小,差異有統(tǒng)計學意義(t=6.013,7.669,P均0.001)。Avg GCC、Sup GCC、Inf GCC、GCC-GLV、GCC-FLV、Avg RNFL、Sup RNFL和Inf RNFL厚度診斷PPG的AUC值分別為0.910、0.898、0.908、0.983、0.724、0.930、0.918、0.863,其中GCC參數(shù)中診斷效能最高的GCC-GLV與RNFL參數(shù)中的Avg RNFL的AUC值采用z檢驗無統(tǒng)計學差異(P0.05)。結(jié)論 GCC厚度可作為篩選PPG的有效指標。
[Abstract]:Objective to investigate the significance of detecting the thickness of retinal ganglion cell complex (GCC) in macular area in the diagnosis of pre-visual glaucoma (PPG). Self-control and case-control studies were designed. 20 patients with POAG (20 eyes without visual field defect, 20 eyes with POAG group) and 30 cases (30 eyes) with normal control were enrolled in the study. The patients were divided into two groups: PPG group (20 eyes) and POAG group (20 eyes). Methods the thickness of GCC in macular area and the thickness of (RNFL) in retinal nerve fiber layer around optic disc were measured by RTvue SD-OCT. The ability of distinguishing PPG from normal eyes was evaluated by area (AUC) under the receiver's operating characteristic curve. Main measures: thickness of GCC and RNFL and their AUC values. Results the average thickness of GCC in PPG group was higher than that in control group (Avg GCC), GCC, (Sup GCC), (Inf GCC), RNFL, (Avg RNFL), RNFL, (Sup RNFL), RNFL, (Inf RNFL), and the thickness of (Inf RNFL) was thinner than that in control group.
