【摘要】：耳鳴是最常見的耳科疾病,病因眾多,機(jī)制不明,對(duì)耳鳴進(jìn)行一定的分類是提高療效的關(guān)鍵。本文對(duì)聽覺系統(tǒng),聽覺系統(tǒng)外系統(tǒng),心理情緒系統(tǒng),管控系統(tǒng)在耳鳴中的發(fā)病機(jī)制做簡(jiǎn)要綜述,并提出了實(shí)用的耳鳴臨床分類:1.明顯耳部疾病引起的耳鳴,主要和相當(dāng)程度的聽力損失相關(guān);2.耳鳴的管控系統(tǒng)疾病,包括心理因素以及睡眠,女性激素水平、偏頭痛、過敏、可能影響咽鼓管的鼻腔鼻竇病變、咽喉返流、呼吸睡眠暫停(OSAHS)等因素;3.混合性耳鳴為以上兩種因素的混合;4.危險(xiǎn)的耳鳴則要警惕可能的顱內(nèi)病變以及精神疾病;5.原因不明的耳鳴。采用這個(gè)分類對(duì)耳鳴進(jìn)行針對(duì)性的治療,將大大提高耳鳴治療的有效率。
[Abstract]:Tinnitus is the most common otological disease. The etiology and mechanism of tinnitus are unknown. It is the key to improve the curative effect to classify tinnitus. This article briefly reviews the pathogenesis of auditory system, psycho-emotional system and control system in tinnitus, and puts forward a practical clinical classification of tinnitus: 1. Tinnitus caused by obvious ear diseases, mainly associated with a considerable degree of hearing loss; 2. (B) Control of systemic diseases of tinnitus, including psychological factors and sleep, female hormone levels, migraines, allergies, which may affect the nasal and nasal sinus lesions of the eustachian tube, throat reflux, sleep apnea (OSAHS), and other factors; 3. Mixed tinnitus is a mixture of the above two factors; 4. Dangerous tinnitus should be alert to possible intracranial lesions and mental illness; 5. Tinnitus of unknown cause. The treatment of tinnitus by this classification will greatly improve the effective rate of tinnitus treatment.
