【摘要】：目的觀察玻璃體內(nèi)注射雷珠單抗聯(lián)合小梁切除術(shù)及全視網(wǎng)膜光凝(panretinal photocoagulation,PRP)治療新生血管性青光眼(neovascular glaucoma,NVG)的臨床療效。方法選取2013年1月至2014年12月我院收治的NVG患者60例(80眼),均采用玻璃體內(nèi)注射雷珠單抗聯(lián)合小梁切除術(shù)及PRP治療,術(shù)后隨訪1~6個(gè)月,觀察患者術(shù)后一般療效、視力、眼壓及并發(fā)癥等情況。結(jié)果 80眼中手術(shù)完全成功70眼,條件成功6眼,失敗4眼,手術(shù)成功率為95.0%。與術(shù)前比較,術(shù)后1周、1個(gè)月、6個(gè)月視力均有不同程度提高(P0.05)。術(shù)前、術(shù)后1周、1個(gè)月、6個(gè)月眼壓分別為(35.2±2.8)mm Hg(1 k Pa=7.5 mmHg)、(17.4±1.3)mm Hg、(18.2±1.6)mm Hg、(18.4±1.5)mm Hg,與術(shù)前比較,術(shù)后1周、1個(gè)月、6個(gè)月眼壓均下降,差異均有顯著統(tǒng)計(jì)學(xué)意義(均為P0.01)。術(shù)后1周形成功能性濾過泡58眼(82.9%),非功能性濾過泡12眼(17.1%);術(shù)后6個(gè)月形成功能性濾過泡62眼(77.5%),非功能性濾過泡18眼(22.5%)。術(shù)后并發(fā)癥主要包括前房積血、淺前房、前部葡萄膜炎、玻璃體積血等。結(jié)論玻璃體內(nèi)注射雷珠單抗聯(lián)合小梁切除術(shù)及PRP治療NVG能顯著控制眼壓,并發(fā)癥較少。
[Abstract]:Objective to observe the clinical effect of intravitreal injection of Lei Zhu McAb combined with trabeculectomy and total retinal photocoagulation (panretinal photocoagulation,PRP) in the treatment of neovascular glaucoma (neovascular glaucoma,NVG). Methods from January 2013 to December 2014, 60 patients (80 eyes) with NVG were treated with intravitreous injection of Lei Zhu McAb combined with trabeculectomy and PRP. The patients were followed up for 1 to 6 months. The general curative effect and visual acuity of the patients were observed. Intraocular pressure and complications. Results in 80 eyes, the operation was completely successful in 70 eyes, conditional success in 6 eyes, failure in 4 eyes, and the successful rate of operation was 95.0. Compared with preoperative, visual acuity was improved 1 week, 1 month and 6 months postoperatively (P0.05). Intraocular pressure (IOP) was (35.2 鹵2.8) mm Hg () 1 k Pa=7.5 mmHg), (17.4 鹵1.3) mm Hg, (18.2 鹵1.6) mm Hg, (18.4 鹵1.5) mm Hg, before operation, 1 week, 1 month and 6 months after operation, respectively. IOP decreased 1 week, 1 month and 6 months postoperatively (all P 0.01). Functional blebs were found in 58 eyes (82.9%) and non-functional blebs in 12 eyes (17.1%) at 1 week, and functional blebs in 62 eyes (77.5%) and non-functional blebs in 18 eyes (22.5%) 6 months after operation. Postoperative complications include hyphema, superficial anterior chamber, anterior uveitis and vitreous hemorrhage. Conclusion intravitreous injection of Lei Zhu McAb combined with trabeculectomy and PRP can significantly control intraocular pressure (IOP) with less complications.
