阻塞性睡眠呼吸暫停低通氣綜合征患者的牙弓與上氣道形態(tài)變化及相關(guān)性分析

【摘要】:目的:比較阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患者與正常人牙弓及上氣道形態(tài)差異,揭示OSAHS牙弓形態(tài)改變與上氣道容積對應(yīng)變化的關(guān)系。方法:利用上氣道CBCT掃描及口腔模型測量經(jīng)PSG診斷為OSAHS的男性患者(n=22)與正常男性對照(n=19)的牙弓形態(tài)與上氣道容積變化,并行相關(guān)性分析。結(jié)果:OSAHS組上頜牙弓長度與腭弓高度分別大于對照組(均P0.05);OSAHS組鼻咽段、腭咽段截面積及上氣道總?cè)莘e與腭弓高度、上頜牙弓長度呈負相關(guān)(均P0.05),與上頜后段牙弓寬度呈正相關(guān)關(guān)系(P0.05)。結(jié)論:OSAHS患者上頜牙弓形態(tài)的異常與鼻咽段和腭咽段容積的變化有關(guān)聯(lián)。
[Abstract]:Objective: to compare the morphology of dental arch and upper airway in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and normal subjects, and to reveal the relationship between the changes of dental arch morphology and the volume of upper airway in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods: CBCT scanning of upper airway and oral model were used to measure the changes of dental arch shape and upper airway volume in male patients diagnosed as OSAHS by PSG (nnm22) and normal controls (nf19), and the correlation analysis was performed. Results: the length of maxillary arch and the height of palatal arch in OSAHS group were higher than those in control group (P0.05). In OSAHS group, the nasopharyngeal segment, palatopharynx sectional area and total volume of upper airway were negatively correlated with the height of palatal arch and length of maxillary arch (P0.05), and positively correlated with the width of maxillary posterior arch (P0.05). Conclusion: the abnormal shape of maxillary arch is associated with the volume change of nasopharynx and palatopharynx in OSAHS patients.
