急進(jìn)性后部型早產(chǎn)兒視網(wǎng)膜病的危險(xiǎn)因素分析

【摘要】:目的分析急進(jìn)性后部型早產(chǎn)兒視網(wǎng)膜病(AP-ROP)的危險(xiǎn)因素。方法采用回顧性病例對(duì)照研究方法 ,納入2012年3月至2014年9月在廣東省婦幼保健院住院的55例AP-ROP先證者和110例普通ROP患兒,兩者按1∶2進(jìn)行配比,匹配條件:普通ROP患兒、入院時(shí)間最接近,收集、整理資料。結(jié)果胎齡32周(χ~2=8.584),出生體質(zhì)量1 250 g(χ~2=3.960),吸氧(χ~2=7.015),吸氧≥14 d(χ~2=22.273),機(jī)械通氣(χ~2=10.104),機(jī)械通氣≥14 d(χ~2=12.579),使用PS(χ~2=10.942),新生兒貧血(χ~2=8.462),輸血≥2次(χ~2=41.734),RDS(χ~2=11.733),新生兒肺炎(χ~2=25.536),呼吸暫停(χ~2=23.468),BPD(χ~2=8.319),顱內(nèi)出血(χ~2=6.478),代謝性酸中毒(χ~2=8.317),視網(wǎng)膜出血(χ~2=10.188)與AP-ROP顯著相關(guān)(P0.05)。在單因素分析基礎(chǔ)上運(yùn)用條件Logistic回歸分析,輸血≥2次(OR:14.956)和呼吸暫停(OR:7.590)是AP-ROP的獨(dú)立危險(xiǎn)因素。結(jié)論 AP-ROP的發(fā)生與出生胎齡小、出生體重低、氧療及相關(guān)并發(fā)癥(如呼吸窘迫綜合征、視網(wǎng)膜出血等)有關(guān),輸血和呼吸暫停是AP-ROP的獨(dú)立危險(xiǎn)因素。
