鈦質(zhì)人工聽骨在一期鼓室成形術(shù)中的應(yīng)用

【摘要】:目的研究鈦質(zhì)人工聽骨植入在一期鼓室成形術(shù)中的療效。方法對3 1例用鈦質(zhì)人工聽骨行一期鼓室成形術(shù)的患者,隨訪3-6個月。通過計算術(shù)前術(shù)后純音測聽得出氣骨導(dǎo)閾值的平均值,比較手術(shù)后氣導(dǎo)的恢復(fù)情況和手術(shù)前后的氣骨導(dǎo)差。結(jié)果 3 1例患者術(shù)后鼓膜愈合良好,未見人工聽骨排異反應(yīng),及聽骨脫出,有1例在術(shù)后2個月時發(fā)生感染。31耳術(shù)前氣導(dǎo)聽力30.00~88.75dB,平均為(59.27±15.67)dB,術(shù)后氣導(dǎo)平均聽力為(43.10±14.64)dB,與術(shù)前相比,差異有統(tǒng)計學(xué)意義(P0.05)。術(shù)前骨導(dǎo)聽力為10.00~43.75dB,平均為(23.06±9.18)dB,術(shù)后骨導(dǎo)平均聽力為(22.82±7.49)dB,與術(shù)前相比,差異無統(tǒng)計學(xué)意義(P0.05)。術(shù)前氣骨導(dǎo)差(ABG)為11.25~62..5dB,平均為(36.21±11.88)dB,術(shù)后ABG平均為(19.40±12.92)dB,二者比較,差異有統(tǒng)計學(xué)意義(p0.05)。術(shù)后ABG≤20dB且ABG較術(shù)前縮小15dB為聽力提高有效,共有13耳,有效率為41.94%(13/31)。結(jié)論鈦質(zhì)人工聽骨植入在一期鼓室成形術(shù)中可以取得滿意的聽力效果,術(shù)后并發(fā)癥少,排除率低,在有聽骨鏈破壞行Ⅰ期鼓室成形術(shù)中值得推廣應(yīng)用。

Methods 31 cases of primary tympanoplasty with titanium artificial ossicle were followed up for 3-6 months. The mean value of air-bone conduction threshold was obtained by preoperation and postoperative pure tone audiometry, and the recovery of air conduction and the difference of air-bone conductance before and after operation were compared. Results the tympanic membrane healed well in 31 patients after operation, and there was no artificial ossicular rejection and ossicular prolapse.
