IOL-Master在不同公式下對(duì)軸性高度近視眼術(shù)后屈光準(zhǔn)確度評(píng)估

【摘要】:目的應(yīng)用IOL-Master通過(guò)不同人工晶狀體計(jì)算公式,分析軸性高度近視眼患者行白內(nèi)障手術(shù)后目標(biāo)屈光和實(shí)際屈光的符合程度,評(píng)價(jià)不同人工晶狀體計(jì)算公式的準(zhǔn)確度。方法收集2013年1月至2014年4月在我院行白內(nèi)障手術(shù)的軸性高度近視患者66例(100眼),將其按眼軸長(zhǎng)度(axial length,AL)分為三組,組1為26 mm≤AL28 mm(54眼),組2為28≤AL30 mm(28眼),組3為AL≥30 mm(18眼);按角膜曲率(K)分為四組,組1為≤42.00 D(17眼),組2為42.00~44.00 D(27眼),組3為44.00~46.00 D(48眼),組4為46.00 D(8眼)。術(shù)前均采用Haigis、SRK/T、Holladay I及Hoffer Q公式進(jìn)行人工晶狀體度數(shù)計(jì)算。應(yīng)用IOL-Master的部分相干干涉測(cè)量(partial coherence interferometry,PCI)技術(shù)計(jì)算人工晶狀體度數(shù)。分析Haigis、SRK/T、Holladay I及Hoffer Q四種計(jì)算公式所預(yù)估的術(shù)后目標(biāo)屈光度與術(shù)后6個(gè)月的實(shí)際屈光度符合程度。結(jié)果按AL分組,組1中,四種公式對(duì)術(shù)后實(shí)際屈光的預(yù)測(cè)準(zhǔn)確度一樣,四種方法之間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);組2中,SRK/T公式的目標(biāo)屈光與實(shí)際屈光的差值最小,準(zhǔn)確度最高,四種方法之間比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);組3中,Haigis公式的目標(biāo)屈光與實(shí)際屈光的差值最小,準(zhǔn)確度最高,四種方法之間比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。按角膜曲率分組,各組中四種方法之間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論利用IOL-Master對(duì)長(zhǎng)眼軸進(jìn)行人工晶狀體度數(shù)計(jì)算時(shí),Haigis公式具有相對(duì)穩(wěn)定的準(zhǔn)確度,而隨著患者眼軸的增加,準(zhǔn)確度逐漸下降。
