干擾素-γ、白介素-4及白介素-17在鼻息肉中的表達(dá)及其意義

【摘要】:目的:采用HE染色法觀察人鼻息肉中嗜酸性粒細(xì)胞(EOS)及炎癥細(xì)胞的浸潤(rùn)情況,采用免疫組化SABC染色法觀察IFN-γ、IL-4及IL-17在鼻息肉中的表達(dá)情況。探討Th1、Th2及Th17細(xì)胞在鼻息肉發(fā)生發(fā)展中的作用。 研究對(duì)象及方法: 對(duì)象:隨機(jī)選取2009年12月-2011年12月在我院耳鼻咽喉科住院做鼻內(nèi)鏡手術(shù)的鼻息肉患者30例,作為實(shí)驗(yàn)組。其中男17例、女13例,年齡17-60歲,平均年齡41.7歲。鼻息肉切除術(shù)后所有鼻息肉標(biāo)本均經(jīng)2位以上病理科副主任醫(yī)師核實(shí),并且排除既往有鼻息肉病史、變應(yīng)性鼻炎及哮喘病史,排除其他變態(tài)反應(yīng)及全身病史;鼻中隔矯正術(shù)患者正常下鼻甲組織20例,作為正常對(duì)照組。其中男12例、女8例,年齡18-53歲,平均年齡31.6歲。并排除有變應(yīng)性鼻炎史及其他全身病史。 方法:全部標(biāo)本經(jīng)10%的福爾馬林溶液固定,常規(guī)石蠟包埋,切片后做HE染色和免疫組化SABC染色。HE染色下觀察嗜酸性粒細(xì)胞及炎癥細(xì)胞浸潤(rùn)情況,兩種組織間的數(shù)據(jù)均以均數(shù)±標(biāo)準(zhǔn)差(x±S)表示,行t檢驗(yàn)。采用免疫組織化學(xué)法檢測(cè)兩種組織中IFN-γ、IL-4和IL-17在的表達(dá)情況。三種細(xì)胞因子在兩種組織中的表達(dá)均以均數(shù)±標(biāo)準(zhǔn)差(x±S)表示,行t檢驗(yàn)。用Pearson直線相分析EOS與三種細(xì)胞因子之間的相關(guān)性。 結(jié)果: 1.人鼻息肉中嗜酸性粒細(xì)胞浸潤(rùn)數(shù)明顯高于正常下鼻甲組(p0.01) 2.IFN-γ在鼻息肉中的表達(dá)與正常下鼻甲組之間無(wú)明顯差異(p0.05)。 3.IL-4在鼻息肉中的表達(dá)明顯高于正常下鼻甲組(p0.01)。 4.IL-17在鼻息肉中的表達(dá)明顯高于正常下鼻甲組(p0.01)。 5.在鼻息肉組織中EOS與IFN-γ缺乏相關(guān),相關(guān)系數(shù)r值=-0.206(p0.05)。 6.在鼻息肉組織中EOS與IL-4呈正相關(guān),相關(guān)系數(shù)r值=0.392(p0.05)。 7.在鼻息肉組織中EOS與IL-17呈正相關(guān),相關(guān)系數(shù)r值=0.421(p0.05)。 結(jié)論:鼻息肉中IL-4和IL-17表達(dá)均明顯增強(qiáng),提示鼻息肉中Th2/Th17細(xì)胞占優(yōu)勢(shì)。鼻息肉中可見大量的EOS浸潤(rùn),IL-4和IL-17可能參與了EOS的趨化和聚集,并在鼻息肉形成過(guò)程中發(fā)揮重要作用。
[Abstract]:Objective: to observe the infiltration of eosinophil (EOS) and inflammatory cells in human nasal polyps by HE staining, and to observe the expression of IFN- 緯, IL-4 and IL-17 in nasal polyps by immunohistochemical SABC staining. To investigate the role of Th1,Th2 and Th17 cells in the development of nasal polyps. Objects and methods: thirty patients with nasal polyps who were hospitalized in our department of otorhinolaryngology from December 2009 to December 2011 were selected as experimental group. There were 17 males and 13 females, aged 17-60 years, with an average age of 41.7 years. All nasal polyps were verified by more than 2 deputy chief pathologists after resection of nasal polyps, and the history of nasal polyps, allergic rhinitis and asthma, other allergic reactions and systemic history were excluded. 20 cases of normal inferior turbinate were used as normal control group. There were 12 males and 8 females, aged 18-53 years, with an average age of 31.6 years. A history of allergic rhinitis and other systemic diseases was excluded. Methods: all the specimens were fixed with 10% formalin solution and embedded in paraffin. The infiltration of eosinophils and inflammatory cells was observed by HE staining and immunohistochemical SABC staining. The data between the two tissues were expressed as mean 鹵standard deviation (x 鹵S), and t test was performed. Immunohistochemical method was used to detect the expression of IFN- 緯, IL-4 and IL-17 in the two tissues. The expression of three cytokines in both tissues was expressed as mean 鹵standard deviation (x 鹵S), and t-test was performed. The correlation between EOS and three cytokines was analyzed by Pearson linear phase. Results: 1. The number of eosinophil infiltration in human nasal polyps was significantly higher than that in normal inferior turbinate group (p0.01). 2. The expression of IFN- 緯 in nasal polyp was not significantly different from that in normal inferior turbinate group (p0.05). The expression of 3.IL-4 in nasal polyps was significantly higher than that in normal inferior turbinate group (p 0.01). The expression of 4.IL-17 in nasal polyps was significantly higher than that in normal inferior turbinate group (p 0.01). 5. There was no correlation between EOS and IFN- 緯 in nasal polyps (r =-0.206). 6. There was a positive correlation between EOS and IL-4 in nasal polyps (r = 0.392). 7. There was a positive correlation between EOS and IL-17 in nasal polyps (r = 0.421). Conclusion: the expression of IL-4 and IL-17 in nasal polyps was significantly increased, suggesting that Th2/Th17 cells were dominant in nasal polyps. A large number of EOS infiltrates can be seen in nasal polyps. IL-4 and IL-17 may be involved in the chemotaxis and aggregation of EOS and play an important role in the formation of nasal polyps.
