【摘要】：背景:臨床顯示自體硬腭黏膜對瞼板結(jié)膜有較好的替代作用。目的:分析自體游離硬顎黏膜用于眼瞼重建的臨床效果。方法:納入180例(180眼)眼瞼缺損患者,其中男122例,女58例,年齡10-68歲,均進行自體硬顎黏膜移植修復治療,眼瞼軟組織缺損部位采用皮瓣進行修復,治療后隨訪觀察患者情況及硬腭黏膜情況。結(jié)果與結(jié)論:所有患者均隨訪6個月以上。(1)治療后3個月內(nèi),患者硬腭黏膜移植區(qū)及結(jié)膜囊內(nèi)產(chǎn)生黏性分泌物,在使用抗生素滴眼液后得到減輕或消失;(2)硬腭黏膜移植片在隨訪的1年內(nèi)全部存活,未發(fā)生感染或脫落情況,移植片與接受區(qū)域之間已有血管長入并局部已與原有組織血管吻合,未發(fā)生排斥反應;(3)132例患者自體硬腭黏膜移植片發(fā)生不同程度收縮,但眼部正常功能未受到影響;(4)住院期間眼部疼痛等不適癥狀已有明顯減輕,創(chuàng)面未見感染或愈合不良的情況發(fā)生;(5)隨訪期間,8例發(fā)生輕度下瞼外翻,3例輕度眼瞼閉合不全,其余患者眼瞼閉合情況及眼瞼開閉功能均未見明顯異常;(6)除了8例眼球缺失患者,其余患者雙眼眼瞼的長度差及高度差均在2 mm內(nèi);(7)結(jié)果表明,自體硬腭黏膜重建眼瞼效果良好,具有較好的相容性。
[Abstract]:Background: autologous hard palate mucosa is a good substitute for tarsal conjunctiva. Objective: to analyze the clinical effect of self-free hard jaw mucosa in eyelid reconstruction. Methods: 180 cases (180 eyes) of eyelid defects were included, including 122 males and 58 females, aged 10-68 years. All of them were treated with autogenous hard jaw mucosa transplantation, and the soft tissue defects of eyelids were repaired with flaps. Patients and hard palate mucosa were followed up after treatment. Results and conclusion: all the patients were followed up for more than 6 months. (1) within 3 months after treatment, mucous secretions were produced in the hard palate mucosa transplantation area and conjunctival sac, which were alleviated or disappeared after the use of antibiotic eye drops. (2) all the grafts of hard palate mucosa survived within one year, no infection or shedding occurred, and there was no rejection. (3) in 132 patients, autologous hard palate mucosa grafts contracted to varying degrees, but normal ocular function was not affected. (4) the symptoms of ocular pain and other discomfort have been obviously alleviated during hospitalization, and no infection or poor healing occurred in the wound. (5) during the follow-up period, 8 cases had mild ectropion, 3 cases had mild eyelid insufficiency, and the other patients had no obvious abnormality in eyelid closure and eyelid opening and closing function. (6) except for 8 cases of eye loss, the difference in length and height of eyelid in other patients was within 2 mm. (7) the results showed that the reconstruction of eyelid with autologous hard palate mucosa was effective and had good compatibility.
