【摘要】：目的:比較單純使用翼狀胬肉切除術(shù)和聯(lián)合自體角膜緣干細(xì)胞移植術(shù)的臨床效果。方法:將94例原發(fā)性翼狀胬肉進(jìn)行期患者隨機分為兩組各47例患者(47眼)。對照組行單純翼狀胬肉切除術(shù);觀察組行翼狀胬肉切除+自體角膜緣干細(xì)胞移植術(shù)。測量并比較兩組術(shù)前,術(shù)后1周、1個月及3個月的淚膜破裂時間、角膜散光度、術(shù)后3個月后的復(fù)發(fā)情況、術(shù)后并發(fā)癥及術(shù)后主觀癥狀。結(jié)果:治療后,觀察組患者復(fù)發(fā)率6.38%,顯著低于對照組患者的23.4%(P0.05)。兩組患者淚膜破裂時間及散光度改善程度均與治療前構(gòu)成統(tǒng)計學(xué)差異,但對照組淚膜破裂時間在術(shù)后1周下降幅度高于觀察組(P0.05)。兩組術(shù)后并發(fā)癥無顯著差異;但對照組存在主觀不適的患者數(shù)量各時期均高于觀察組(P0.05)。結(jié)論:兩術(shù)聯(lián)用治療翼狀胬肉安全性高、療效較好,值得推廣。
[Abstract]:Objective: to compare the clinical effects of pterygium excision combined with limbal stem cell autotransplantation. Methods: 94 patients with primary pterygium were randomly divided into two groups (47 eyes). The control group was treated with simple pterygium excision and the observation group with limbal stem cell transplantation. The tear film rupture time, corneal astigmatism, recurrence after 3 months, postoperative complications and subjective symptoms were measured and compared between the two groups before, 1 week, 1 month and 3 months after operation. Results: after treatment, the recurrence rate of the patients in the observation group was 6.38, which was significantly lower than that in the control group (23.4%) (P0.05). The time of tear film rupture and the degree of improvement of astigmatism in the two groups were significantly different from those before treatment, but the time of tear film rupture in the control group was significantly lower than that in the observation group at 1 week after operation (P0.05). There was no significant difference in postoperative complications between the two groups, but the number of patients with subjective discomfort in the control group was higher than that in the observation group (P0.05). Conclusion: the combined treatment of pterygium is safe and effective, and is worth popularizing.
