【摘要】：目的探討青少年屈光參差患者配戴角膜塑形鏡3年后眼軸、中央角膜厚度及角膜內(nèi)皮細(xì)胞密度的變化。設(shè)計(jì)回顧性病例系列。研究對(duì)象2011-2013年在北京同仁驗(yàn)光配鏡中心驗(yàn)配角膜塑形鏡且連續(xù)戴鏡3年以上、按時(shí)復(fù)診的近視性屈光參差青少年患者21例(42眼)。方法依近視度數(shù)將患眼分為高度數(shù)眼組(高于-3.00 D)及低度數(shù)眼組(低于等于-3.00 D),觀察并比較戴鏡前及戴鏡6個(gè)月、1年、2年、3年后各組眼軸、中央角膜厚度及角膜內(nèi)皮細(xì)胞密度等參數(shù)的變化。主要指標(biāo)眼軸、中央角膜厚度、角膜內(nèi)皮細(xì)胞密度。結(jié)果戴鏡前及戴鏡6個(gè)月、1年、2年、3年后高度數(shù)眼組眼軸分別為(25.16±0.78)mm、(25.27±0.81)mm、(25.43±0.91)mm、(25.50±0.85)mm和(25.64±0.87)mm,低度數(shù)眼組眼軸分別為(24.13±0.99)mm、(24.38±0.96)mm、(24.49±1.00)mm、(24.71±0.94)mm和(24.88±1.01)mm,雙眼眼軸均隨時(shí)間增長(zhǎng)(F=48.2;P=0.000),高度數(shù)眼組眼軸更長(zhǎng)(F=10.186;P=0.003)。各時(shí)間點(diǎn)眼軸差異量分別為(1.03±0.45)mm、(0.90±0.40)mm、(0.85±0.39)mm、(0.79±0.41)mm及(0.76±0.44)mm,戴鏡后雙眼眼軸差異量逐漸縮小(F=9.494;P=0.000)。戴鏡前及戴鏡6個(gè)月、1年、2年、3年后高度數(shù)眼組角膜內(nèi)皮細(xì)胞密度分別為(3362.57±278.73)個(gè)/mm~2、(3393.67±325.88)個(gè)/mm~2、(3333.76±288.72)個(gè)/mm~2、(3276.90±240.49)個(gè)/mm~2及(3259.71±357.41)個(gè)/mm~2,低度數(shù)眼組角膜內(nèi)皮細(xì)胞密度分別為(3365.62±352.75)個(gè)/mm~2、(3387.62±363.57)個(gè)/mm~2、(3314.05±270.12)個(gè)/mm~2、(3290.71±327.64)個(gè)/mm~2及(3307.05±357.12)個(gè)/mm~2,隨著戴鏡時(shí)間延長(zhǎng),雙眼角膜內(nèi)皮細(xì)胞密度呈下降趨勢(shì)(F=4.386;P=0.006),雙眼間比較無(wú)統(tǒng)計(jì)學(xué)差異(F=0.007,P=0.932)。戴鏡前及戴鏡6個(gè)月、1年、2年、3年后高度數(shù)眼組中央角膜厚度分別為(548.81±28.34)μm、(531.43±33.30)μm、(526.05±30.84)μm、(523.38±27.54)μm和(525.29±27.92)μm,低度數(shù)組眼中央角膜厚度分別為(549.76±28.72)μm、(536.90±26.51)μm、(534.24±27.67)μm、(534.19±25.84)μm及(531.76±27.16)μm,戴鏡后6個(gè)月中央角膜厚度下降,以后無(wú)變化(F=56.843;P=0.000)。結(jié)論青少年近視性屈光參差患者長(zhǎng)期配戴角膜塑形鏡后雙眼眼軸差距縮小。角膜塑形效果于戴鏡6個(gè)月內(nèi)穩(wěn)定。
[Abstract]:Objective to investigate the changes of corneal axis, central corneal thickness and corneal endothelial cell density in juvenile anisometropia patients after wearing corneal plastic lens for 3 years. Design retrospective case series. Participants 21 cases (42 eyes) of myopic anisometropia were examined in Beijing Tongren optometry Center from 2011 to 2013. Methods according to the degree of myopia, the eyes were divided into high degree group (> -3.00D) and low degree eye group (< -3.00 D),) and compared with each group after 6 months, 1 year, 2 years and 3 years after wearing mirror.
