【摘要】：目的:評(píng)估中藥湯劑潤(rùn)明湯聯(lián)合人工淚液治療干眼的臨床療效,探尋持久有效的治療氣陰兩虛型干眼的方法。材料與方法:本試驗(yàn)收集2016年7月到2016年12月就診于沈陽(yáng)何氏眼科醫(yī)院符合干眼診斷標(biāo)準(zhǔn)的64例(128只眼)患者,中醫(yī)辨證屬于氣陰兩虛型。采用隨機(jī)對(duì)照的實(shí)驗(yàn)方法,將64例(128只眼)患者隨機(jī)分成治療組32例(64只眼)和對(duì)照組32例(64只眼)。治療組采用院內(nèi)制劑潤(rùn)明湯水煎服,日兩次口服,每次100ml,同時(shí)聯(lián)合人工淚液(聚乙二醇滴眼液),日5次點(diǎn)雙眼。對(duì)照組單純?nèi)斯I液(聚乙二醇滴眼液),日5次點(diǎn)雙眼。分別觀察治療前、治療后4周干眼相關(guān)評(píng)價(jià)指標(biāo)(干眼主觀癥狀評(píng)分、SIt、BUT、FL)。結(jié)果:1、治療4周后,治療組總體有效率為90.32%,明顯高于對(duì)照組的76.67%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2、治療4周后,兩組干眼患者主觀癥狀總評(píng)分及各項(xiàng)評(píng)分、角膜熒光素染色評(píng)分較治療前有所降低(P0.01);治療組SIt、BUT較治療前有所增加(P0.01);對(duì)照組BUT較治療前有所增加(P0.01),SIt與治療前比較無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。3、治療4周后,治療組干眼主觀癥狀(干澀感、視疲勞、異物感)評(píng)分降低程度、SIt、BUT增加量均優(yōu)于對(duì)照組(P0.05)。4、治療4周后,治療組與對(duì)照組干眼主觀癥狀(眼紅、眼痛)評(píng)分、FL評(píng)分降低程度無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論:單純?nèi)斯I液(聚乙二醇滴眼液)治療干眼以及潤(rùn)明湯聯(lián)合人工淚液治療干眼均能在一定程度上改善患者的主觀癥狀以及客觀指標(biāo),但聯(lián)合治療組效果明顯優(yōu)于單純?nèi)斯I液組,能從根本上緩解干眼患者的不適癥狀。
[Abstract]:Objective: to evaluate the clinical efficacy of Runming decoction combined with artificial tear in the treatment of dry eyes and to explore a lasting and effective method for treating dry eyes with deficiency of Qi and Yin. Materials and methods: from July 2016 to December 2016, 64 patients (128 eyes) who met the criteria of dry eye diagnosis in Shenyang Ho's Eye Hospital were collected. 64 patients (128 eyes) were randomly divided into treatment group (n = 32, n = 64) and control group (n = 32, n = 64). In the treatment group, hospital preparation Runming decoction was taken twice a day, 100ml per time, combined with artificial tear (polyethylene glycol eye drops), 5 times a day point eyes. In the control group, artificial tears (polyethylene glycol eye drops) were used five times a day. The relative indexes of dry eye (dry eye subjective symptom score, SIt,BUT,FL) were observed before and 4 weeks after treatment. Results: 1. After 4 weeks of treatment, the total effective rate of the treatment group was 90.32, which was significantly higher than that of the control group (76.67), the difference was statistically significant (P0.05). After 4 weeks of treatment, the total score of subjective symptoms and the scores of the two groups were higher than that of the control group. Corneal fluorescein staining score was lower than that before treatment (P0.01), SIt,BUT in treatment group was higher than that before treatment (P0.01), BUT in control group was higher than that before treatment (P0.01), SIt was not significantly different from that before treatment (P0.05). 3. After 4 weeks of treatment, the subjective symptoms of dry eye in treatment group were dry and astringent. Visual fatigue, foreign body sensation) score decreased, SIt,BUT increased better than the control group (P0.05). 4 weeks after treatment, the treatment group and control group dry eye subjective symptoms (eye red, eye pain) score, FL score decreased no significant difference (P0.05). Conclusion: the treatment of dry eye with pure artificial tear (polyethylene glycol eye drops) and Runming decoction combined with artificial tear can improve the subjective symptoms and objective indexes of the patients to some extent. But the effect of combined treatment was better than that of artificial tear group, which could relieve the uncomfortable symptoms of dry eye patients.
