【摘要】：目的： 在第一部分實驗中,為了尋求一種可替代多導睡眠觀測的經(jīng)濟可行而又具備可以提供客觀數(shù)據(jù)的觀察方法,測試音頻采集結合計算機軟件音頻分析方法對單純性鼾癥患者鼾聲數(shù)據(jù)監(jiān)測的可行性；同時觀測單純性鼾癥患者鼾癥夜間睡眠期鼾癥行為特點,以為下一步實驗提供客觀可行的觀察切入點。 在第二部分實驗中,根據(jù)第一部分實驗的研究結果,通過隨機對照臨床試驗,并擬采用音頻分析方法,觀察推拿療法對單純性鼾癥患者夜間睡眠期鼾癥行為的影響。 方法： 在第一部分實驗中,通過對10例單純性鼾癥患者進行為期2周的睡眠期鼾聲音頻采集,得到數(shù)字化音頻文件；通過計算機軟件分析音頻波形,得到鼾聲強度、打鼾次數(shù)等數(shù)據(jù),將數(shù)據(jù)錄入記錄表；測試音頻采集結合計算機軟件音頻分析方法對單純性鼾癥患者鼾聲數(shù)據(jù)監(jiān)測的可行性；利用統(tǒng)計學分析方法,分析基于音頻分析得出的單純性鼾癥患者鼾癥夜間睡眠期鼾癥行為特點,及確定實際可測量數(shù)據(jù)。 在第二部分實驗中,根據(jù)第一部分實驗的研究結果,進行下一階段的臨床隨機對照實驗。對20例單純性鼾癥患者按隨機數(shù)字分組法分為觀察組和對照組；觀察組予以推拿手法治療,對照組按辨證采用中藥方劑治療；兩組患者均在治療前、治療過程的2周和治療后2周中采用錄音工具對其進行睡眠期錄音；完成全過程后回收錄音數(shù)據(jù),利用計算機軟件對其進行音頻處理和分析,將數(shù)據(jù)錄入記錄表格。通過統(tǒng)計學計算方法,分析推拿對單純性鼾癥的近期、中期及遠期療效。 成果： 在第一部分實驗中,證實了音頻分析用于對單純性鼾癥患者觀測中的可行性,對已發(fā)現(xiàn)的不足提出了初步解決方法；同時確定對單純性鼾癥觀測時間為入睡后第2至第3小時,前期及長期觀察日期為3天,并以鼾聲強度和打鼾次數(shù)作為下一步實驗中觀察點。 在第二部分實驗中,觀察組患者鼾聲強度降低及打鼾次數(shù)減少優(yōu)于對照組,且P0.05,差異具統(tǒng)計學意義；提示推拿治療單純性鼾癥有較好的近期療效。在中遠期療效觀察中,兩組患者的鼾聲強度及打鼾次數(shù)均有明顯回升,組間比較,P0.05,差異無統(tǒng)計學意義,提示兩種療法對于單純性鼾癥的中遠期療效均不理想。 結論： 音頻分析可作為單純性鼾癥臨床研究的可行性手段。推拿對單純性鼾癥有較好的近期療效,但中遠期療效不理想；應當結合中藥治療、自我按摩保健和單純性鼾癥健康教育(戒煙戒酒、鍛煉、減輕體重、側臥睡眠等),以取得較滿意的長期療效。
[Abstract]:Objective: in the first part of the experiment, in order to find an economical and feasible alternative to polysomnography and to provide objective data, The feasibility of monitoring the snoring data of patients with simple snoring and the characteristics of snoring behavior during nocturnal sleep in patients with simple snoring were studied by using the method of audio acquisition and computer software audio analysis. It provides an objective and feasible entry point for the next experiment. In the second part of the experiment, according to the results of the first part of the experiment, the effects of massage therapy on nocturnal snoring behavior of patients with simple snoring were observed by randomized controlled clinical trial and audio frequency analysis. Methods: in the first part of the experiment, 10 patients with simple snoring were collected snoring audio data during sleep for 2 weeks, and the digital audio files were obtained, and the snoring intensity was obtained by analyzing the audio waveform by computer software. Snoring times and other data, data input into the record table; test audio acquisition combined with computer software audio analysis method for simple snoring patients snoring data monitoring feasibility, using statistical analysis method, The characteristics of snoring behavior during nocturnal sleep in patients with simple snoring were analyzed based on audio frequency analysis, and the actual measurable data were determined. In the second part, according to the results of the first part of the experiment, the next phase of the clinical randomized controlled trial. Twenty patients with simple snoring were randomly divided into observation group and control group. The observation group was treated with massage manipulation, while the control group was treated with traditional Chinese medicine prescription according to syndrome differentiation. During the two weeks of treatment and 2 weeks after treatment, the recording tools were used to record the sleep period of the patients, and the recorded data were recovered after the whole process was completed, and the data were processed and analyzed by the computer software, and the data were recorded in the recording form. The short-term, medium-term and long-term effects of massage on simple snoring were analyzed by statistical calculation. Results: in the first part of the experiment, the feasibility of audio frequency analysis in the observation of patients with simple snoring was confirmed, and the preliminary solution to the problems found was put forward. At the same time, the observation time of simple snoring was determined to be the 2nd to 3rd hour after falling asleep, the early and long-term observation dates were 3 days, and the snoring intensity and snoring times were taken as the observation points in the next experiment. In the second part of the experiment, the snoring intensity and snoring times in the observation group were lower than those in the control group, and the difference was statistically significant (P 0.05), which suggested that massage had a better short-term effect on simple snoring. In the observation of middle and long term curative effect, the snoring intensity and snoring times of the two groups were obviously increased. The difference between the two groups was not statistically significant (P0.05), which suggested that the two treatments were not ideal for the patients with simple snoring. Conclusion: audio frequency analysis can be used as a feasible method for clinical study of simple snoring. Massage has a good short-term effect on simple snoring, but the medium- and long-term effect is not ideal. It should be combined with traditional Chinese medicine treatment, self-massage health care and simple snoring health education (quit smoking and alcohol, exercise, weight loss, Side-lying sleep, etc.), in order to obtain a more satisfactory long-term effect.
