【摘要】：目的研究眼底照相機(jī)外眼照相在斜視篩查中的應(yīng)用及與臨床醫(yī)生檢查的一致性。設(shè)計(jì)前瞻性比較性病例系列。研究對(duì)象北京同仁醫(yī)院眼科屈光門診年齡5歲以上的患者101例。方法采用眼底照相機(jī)照患者歪頭照加九張眼位像,對(duì)所有圖像由一位眼肌專業(yè)中級(jí)職稱醫(yī)生按照盲法原則獨(dú)立進(jìn)行判讀,并與臨床眼位診斷的結(jié)果進(jìn)行一致性檢驗(yàn)。主要指標(biāo)Kappa值、敏感度、特異度。結(jié)果斜視臨床診斷與醫(yī)師對(duì)101例歪頭照加九張眼位像診斷Kappa值為0.91,歪頭照加九張眼位像在斜視診斷中的敏感度為98.6%、特異度100%、誤診率0%、漏診率1.4%。ROC曲線下面積為0.993。結(jié)論眼底照相機(jī)外眼照相在斜視篩查中準(zhǔn)確性好,可以在遠(yuǎn)程眼科診療中推廣。
[Abstract]:Objective to study the application of fundus camera extraocular photography in strabismus screening and its consistency with clinical examination. Design prospective comparative case series. Participants 101 patients over 5 years old in ophthalmic refractive clinic of Tongren Hospital in Beijing. Methods all the images were interpreted by a medial professional ophthalmic doctor in accordance with the principle of blind method, and the results were consistent with the results of clinical eye position diagnosis. Main outcome measures Kappa value, sensitivity, specificity. Results the Kappa value of clinical diagnosis of strabismus and doctor's diagnosis of 101 cases with askew head plus nine eye position images was 0.91, the sensitivity of crooked head and nine eye position images in the diagnosis of strabismus was 98.6, the specificity was 100 and the misdiagnosis rate was 0 and the area under the 1.4%.ROC curve of missed diagnosis rate was 0.993. Conclusion external ophthalmography with fundus camera has good accuracy in strabismus screening and can be popularized in remote ophthalmology diagnosis and treatment.
