【摘要】：目的:探討鼻唇折疊皮瓣在外鼻腫瘤切除后鼻翼全層缺損Ⅰ期修復(fù)中的應(yīng)用。方法:外鼻腫瘤15例,腫瘤切除后鼻翼全層缺損,Ⅰ期用鼻唇尖端折疊皮瓣修復(fù)鼻翼缺損。術(shù)前探測鼻唇區(qū)血管分布以及該區(qū)域血管穿支淺出點,按照缺損的區(qū)域和大小設(shè)計鼻唇尖端折疊皮瓣,根據(jù)缺損修復(fù)要求修剪皮瓣,使皮瓣最終成為厚薄適宜、鼻外被皮膚和鼻內(nèi)襯里的局部皮瓣,旋轉(zhuǎn)轉(zhuǎn)移修復(fù)鼻翼全層缺損。結(jié)果:15例患者皮瓣全部存活,術(shù)后隨訪3個月~3年,瘢痕不明顯,鼻翼修復(fù)形態(tài)滿意,鼻腔內(nèi)痂皮少,修復(fù)鼻翼感覺良好。結(jié)論:鼻唇折疊皮瓣具有血供好、蒂長、皮瓣厚度適宜、外被皮膚、內(nèi)有襯里的優(yōu)點,修復(fù)后鼻翼形態(tài)滿意,鼻腔內(nèi)干痂少,并保留了外鼻的感覺功能。鼻唇折疊皮瓣是外鼻腫瘤切除后鼻翼全層缺損Ⅰ期修復(fù)較為理想的方法。
[Abstract]:Objective: to investigate the application of nasolabial folding flap in the repair of full-thickness alar defect after resection of external nasal tumor. Methods: 15 cases of external nasal neoplasms were resected with full-thickness defect of the nasal alar. In the first stage, the nasal alar defect was repaired with the tip of the nasal lip flap. Before operation, the distribution of the vessels in the nasolabial region and the superficial point of the perforating branches in the region were detected. The flap was designed according to the area and size of the defect, and the flap was pruned according to the repair requirements of the defect, so that the flap would eventually become thick and suitable. External nasal skin and nasal lining of the local flap, rotating transfer repair of the full-thickness defect of the nasal wing. Results all the 15 cases survived, followed up for 3 months to 3 years, the scar was not obvious, the shape of nasal wing repair was satisfactory, the callus was less in nasal cavity, and the repair of nasal wing felt good. Conclusion: the nasolabial fold flap has the advantages of good blood supply, long pedicle, suitable thickness of flap, outer skin cover, lining inside, satisfactory shape of repair of posterior nasal wing, less callus in nasal cavity, and the sensory function of external nose is preserved. The nasolabial fold flap is an ideal method for the first stage repair of the full-thickness defect of the nasal wing after resection of the external nasal tumor.
