【摘要】：目的檢測干眼患者淚膜脂質(zhì)層厚度(lipid layer thickness,LLT),并分析LLT與眼表體征之間的相關(guān)性。方法納入130例(130眼)干眼患者為本研究研究對象,其中瞼板腺功能障礙(meibomian gland dysfunction,MGD)患者64例,非MGD患者66例,進(jìn)行LLT檢測、淚膜破裂時(shí)間(break-up time,BUT)檢查、角膜熒光素染色(fluorescein staining,FL)、Marx線(Marx line,ML)評分及SchirmerⅠ試驗(yàn)。分析不同年齡段LLT分布情況,及其與其他檢查之間的相關(guān)性。結(jié)果不同年齡段之間LLT差異有統(tǒng)計(jì)學(xué)意義(P=0.007),各年齡段男性與女性之間LLT差異均無統(tǒng)計(jì)學(xué)意義(均為P0.05)。LLT與年齡呈正相關(guān)(r=0.334,P0.001),LLT與性別無顯著相關(guān)性(r=0.107,P=0.226)。LLT與上瞼ML評分(r=0.295,P=0.001)、下瞼ML評分(r=0.233,P=0.008)呈正相關(guān),與BUT、FL及淚液分泌量均無顯著相關(guān)性(均為P0.05)。MGD組LLT與上瞼ML評分、下瞼ML評分呈正相關(guān)(均為r=0.306,P=0.014),與其他檢查指標(biāo)均無顯著相關(guān)性(均為P0.05)。非MGD組LLT與其他檢查指標(biāo)均無顯著相關(guān)性(均為P0.05)。多元線性回歸分析結(jié)果顯示,干眼患者中,年齡、上瞼ML評分與LLT存在線性關(guān)系(年齡:β=0.254,P=0.005;上瞼ML評分:β=0.207,P=0.022),在MGD組中,僅年齡與LLT存在線性關(guān)系(β=0.382,P=0.002)。MGD組上瞼ML評分及下瞼ML評分均高于非MGD組(均為P0.001)。結(jié)論干眼患者LLT與年齡、ML評分呈正相關(guān)。淚膜LLT檢測作為干眼診斷的輔助檢查時(shí),需結(jié)合年齡等影響因素進(jìn)行綜合分析。
