【摘要】：目的分析鼻-鼻竇良、惡性腫瘤的臨床特征,以提高其臨床診斷準(zhǔn)確性。方法對(duì)經(jīng)病理證實(shí)的258例鼻-鼻竇腫瘤患者的臨床資料進(jìn)行回顧性分析并對(duì)比良、惡性腫瘤的臨床差異。結(jié)果鼻-鼻竇腫瘤臨床癥狀表現(xiàn)多樣。鼻出血或涕中帶血、頭痛、面部麻木或疼痛、眼痛或溢淚所占比率比較,惡性腫瘤患者明顯高于良性腫瘤患者(P0.05);行手術(shù)治療的196例患者中,良、惡性腫瘤術(shù)前石蠟病檢與術(shù)后石蠟病檢符合率相比,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。行手術(shù)治療的63例惡性腫瘤中,共行232個(gè)切緣冰凍病檢,與術(shù)后石蠟冰凍病檢符合率為98.70%(229/232)。34例術(shù)中冰凍與術(shù)后石蠟病檢符合率為91.18%(31/34),其中行術(shù)中即時(shí)冰凍6例,66.67%(4/6)的患者與術(shù)前石蠟活檢診斷不相符,從良性變?yōu)閻盒�。結(jié)論鼻-鼻竇腫瘤臨床表現(xiàn)缺乏特異性,術(shù)中快速冰凍病檢有利于提高診斷率;惡性腫瘤的切緣應(yīng)行連續(xù)冰凍病檢。
[Abstract]:Objective to analyze the clinical features of benign and malignant tumors of nasal and paranasal sinuses and to improve their diagnostic accuracy. Methods the clinical data of 258 patients with naso-paranasal sinus tumor confirmed by pathology were retrospectively analyzed and the clinical differences between benign and malignant tumors were compared. Results the clinical symptoms of nasal-paranasal sinus tumors were varied. The proportion of bleeding, headache, facial numbness or pain, eye pain or tears in patients with nosebleed or snot was significantly higher in patients with malignant tumors than in patients with benign tumors (P0.05). There was no significant difference in the coincidence rate between preoperative and postoperative paraffin detection of malignant tumors (P0.05). Of 63 cases of malignant tumors treated by operation, 232 were examined with frozen margin. The coincidence rate was 98.70% (229 / 232). The coincidence rate of intraoperative freezing and postoperative paraffin was 91.18% (31 / 34). Among them, 66.67% (4 / 6) of the patients who underwent intraoperative immediate freezing did not agree with the diagnosis of preoperative paraffin biopsy, from benign to malignant. Conclusion the clinical manifestations of naso-paranasal sinus tumors are lack of specificity, and rapid intraoperative frozen examination is helpful to improve the diagnostic rate, and the incised margin of malignant tumors should be examined continuously.
