【摘要】：目的：評(píng)價(jià)鼻內(nèi)鏡下淚囊鼻腔吻合聯(lián)合淚囊支架植入術(shù)治療慢性淚囊炎的遠(yuǎn)期臨床效果。 方法：將患者隨機(jī)分成兩組，研究組26例（33眼）患者采取鼻內(nèi)鏡下淚囊鼻腔吻合聯(lián)合淚囊支架植入術(shù)，同時(shí)在支架周?chē)钊朊罉?lè)膠固定。對(duì)照組23例（28眼）患者采用單純的鼻內(nèi)鏡下淚囊鼻腔吻合術(shù)，術(shù)中不應(yīng)用淚囊支架，術(shù)畢用美樂(lè)膠把粘膜瓣固定。術(shù)后兩組處理相同。隨訪12月。對(duì)比兩組1月、6月、12月的治愈率和造瘺口的面積。 結(jié)果：研究組與對(duì)照組術(shù)后1個(gè)月的治愈率分別為96.96%、92.86%，測(cè)定兩組造瘺口面積分別為30.14±0.23mm~2、30.10±0.64mm~2，經(jīng)統(tǒng)計(jì)學(xué)分析無(wú)顯著性差異（P0.05）；兩組隨訪6個(gè)月時(shí)治愈率分別為90.91%、71.43%；隨訪12個(gè)月時(shí)治愈率分別為84.85%、60.71%，經(jīng)統(tǒng)計(jì)學(xué)分析存顯著性差異（X2=3.889、4.560，P0.05）。兩組術(shù)后6個(gè)月測(cè)量造瘺口面積分別為：23.95±3.07mm~2、12.57±2.26mm~2；術(shù)后12個(gè)月測(cè)量?jī)山M造瘺口面積分別為：20.12±2.34mm~2、10.15±1.85mm~2；經(jīng)統(tǒng)計(jì)學(xué)分析有顯著性差異(t=16.220、18.233,p0.05)。 結(jié)論：鼻內(nèi)窺鏡下淚囊鼻腔吻合聯(lián)合淚囊支架植入術(shù)治療慢性淚囊炎有利于提高術(shù)后遠(yuǎn)期療效，是一種創(chuàng)傷小，，安全有效的新治療方法。
[Abstract]:Objective: to evaluate the long-term clinical effect of endoscopic dacryocystorhinostomy combined with dacryocyst stent implantation in the treatment of chronic dacryocystitis. Methods: the patients were randomly divided into two groups. 26 patients (33 eyes) in the study group were treated with endoscopic dacryocystorhinostomy combined with dacryocyst stent implantation. 23 patients (28 eyes) in the control group were treated with dacryocystorhinostomy under nasal endoscope without dacryocystorhinostomy. The two groups were treated the same after operation. Follow up for 12 months. The cure rate and the area of the fistula were compared between the two groups in January, June and December. Results: the cure rate of the study group and the control group was 96.96 鹵92.86, respectively, and the area of the fistula was 30.14 鹵0.23 mm / 2 respectively. There was no significant difference between the two groups by statistical analysis (P0.05), and the cure rate of the two groups was 90.9171.43 at 6 months follow-up. At 12 months follow-up, the cure rates were 84.85 and 60.71, respectively, and there was significant difference between them (X2P 3.889 4.560 P 0.05). In the 6 months after operation, the area of the fistula orifice in the two groups was: 23.95 鹵3.07mm / 212.57 鹵2.26mm / 2, respectively, and the area of the fistula orifice was 20.12 鹵2.34mm / 210.15 鹵1.85mm / 2 respectively at 12 months after operation. There was a significant difference between the two groups (t16.22018.233p0.05). Conclusion: endoscopic dacryocystorhinostomy combined with dacryocystorhinostomy is a safe and effective new method for the treatment of chronic dacryocystitis.
