三維有限元模型分析OSAHS患者下頜不同前伸位置的舌骨位置變化的研究

【摘要】:目的建立阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患者的上氣道及毗鄰結(jié)構(gòu)三維有限元模型,通過逐步前伸下頜,觀察OSAHS患者舌骨位置的改變及其與下頜前伸量之間的關(guān)系,為下頜前伸類口腔矯治器治療OSAHS的機理研究提供理論依據(jù)。 方法對OSAHS患者上氣道行薄層CT掃描,以DICOM格式輸出數(shù)據(jù),采用Mimics三維建模軟件和Ansys有限元分析軟件建立上氣道及毗鄰結(jié)構(gòu)的三維有限元模型,然后通過加載下頜骨逐步前伸,觀察舌骨位置的變化及規(guī)律。 結(jié)果建立了OSAHS患者上氣道及毗鄰結(jié)構(gòu)的三維有限元模型,定義采用10節(jié)點的solid92四面體單元劃分網(wǎng)格,按要求劃分網(wǎng)格后,,皮質(zhì)骨、松質(zhì)骨、肌肉及氣道各得到的單元數(shù)和節(jié)點數(shù)分別為:562920、50141、336789個單元,544929、84869、303134個節(jié)點。通過逐步前伸下頜骨,舌骨位置向上向前移動, 結(jié)論應(yīng)用螺旋CT技術(shù)聯(lián)合使用Mimics三維建模軟件,Ansys有限元分析軟件建立OSAHS患者上氣道及其毗鄰結(jié)構(gòu)三維有限元模型,模型的幾何相似性較好,使用靈活;通過下頜逐步前伸,對舌骨位置變化進行分析,有效展示下頜骨前伸與舌骨位置變化兩者之間關(guān)系,證實了研究方法的可行性,為下頜前伸矯治器治療OSAHS提供理論依據(jù),同時為后續(xù)研究OSAHS患者上氣道生物力學分析打下良好的基礎(chǔ)。
[Abstract]:Objective to establish a three-dimensional finite element model of upper airway and adjacent structure in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). To provide a theoretical basis for the study of the mechanism of mandibular protruding oral appliance in the treatment of OSAHS. Methods the upper airway and adjacent structures of patients with OSAHS were scanned by thin slice CT scanning, and the data were outputted in DICOM format. The 3D finite element models of upper airway and adjacent structures were established by Mimics and Ansys software, and then the mandible was gradually extended forward by loading. The changes and rules of hyoid position were observed. Results the three-dimensional finite element model of upper airway and adjacent structure of OSAHS patients was established. The 10-node solid92 tetrahedron element was used to mesh the cortex bone and cancellous bone. The number of units and nodes of muscle and airway were respectively 5449299,848699,303134 nodes. The position of hyoid bone was moved upward through the progressive extension of mandible. Conclusion the 3D finite element model of upper airway and adjacent structure of OSAHS patients was established by using Mimics 3D modeling software and Ansys finite element analysis software. The geometric similarity of the model is good, and it is flexible to use. Through the analysis of the hyoid position change through the mandibular progressive extension, the relationship between the mandibular forward extension and the hyoid position change is effectively demonstrated, which proves the feasibility of the research method. It provides a theoretical basis for the treatment of OSAHS with mandibular extension appliance and lays a good foundation for further study on biomechanical analysis of upper airway in patients with OSAHS.
