【摘要】：目的探討非功能損傷手術(shù)對下咽癌的治療效果。方法選取100例下咽癌患者為觀察對象,根據(jù)其手術(shù)方式不同分為保留喉功能組35例和未保留喉功能組65例。觀察兩組患者術(shù)后恢復(fù)情況、生存率、并發(fā)癥發(fā)生率和生活質(zhì)量的差異。結(jié)果保留喉功能組患者的手術(shù)時(shí)間較未保留喉功能組長(P㩳0.01),而兩組患者術(shù)中出血量和住院時(shí)間差異無統(tǒng)計(jì)學(xué)意義(P㧐0.05);保留喉功能組患者吞咽、語言和呼吸恢復(fù)率高于未保留喉功能組(P㩳0.05);保留喉功能組咽瘺、皮瓣壞死發(fā)生率明顯低于未保留喉功能組(P㩳0.05),而兩組患者術(shù)后感染發(fā)生率無統(tǒng)計(jì)學(xué)意義(P㧐0.05);保留喉功能組患者1年和3年生存率明顯高于未保留喉功能組(P㩳0.05);兩組患者手術(shù)前生活質(zhì)量得分比較差異無統(tǒng)計(jì)學(xué)意義;術(shù)后兩組患者的生活質(zhì)量均較手術(shù)前降低,且未保留喉功能組降低更明顯(P㩳0.05)。結(jié)論非功能損傷性手術(shù)對下咽癌有較好的治療效果,可明顯促進(jìn)患者術(shù)后吞咽、語言和呼吸功能的恢復(fù),提高患者術(shù)后生活質(zhì)量。
[Abstract]:Objective to investigate the effect of nonfunctional injury surgery on hypopharyngeal carcinoma. Methods 100 patients with hypopharyngeal carcinoma were selected and divided into two groups according to their different surgical methods: laryngeal preservation group (n = 35) and unpreserved laryngeal function group (n = 65). The difference of postoperative recovery, survival rate, complication rate and quality of life between the two groups was observed. Results the operative time of the patients with laryngeal preservation function was longer than that of the patients without laryngeal function (P0.01), but there was no significant difference between the two groups in the amount of blood loss and the length of hospitalization (P0. 05), and the patients in the group of reserving laryngeal function had swallowing. The recovery rate of language and respiration was higher than that of the group without laryngeal function (P0. 05). The incidence of flap necrosis was significantly lower than that of unpreserved laryngeal function group (P0. 05), but the incidence of postoperative infection in both groups was not statistically significant (P0. 05), the 1-and 3-year survival rate of laryngectomy group was significantly higher than that of non-preserved laryngeal function group (P0. 05). There was no significant difference in the scores of quality of life before operation. The quality of life in the two groups was lower than that before operation, and that in the group without laryngeal function was significantly lower (P0. 05). Conclusion Non-functional injury surgery has a good therapeutic effect on hypopharyngeal carcinoma. It can obviously promote the recovery of swallowing, speech and respiratory function, and improve the quality of life of the patients after operation.
