【摘要】：目的:本研究旨在客觀評價針刺聯(lián)合常規(guī)療法治療前部缺血性視神經病變(AION)的療效。材料與方法:將符合納入標準的60例前部缺血性視神經病變患者,隨機分為實驗組和對照組,對于實驗組患者采用針刺聯(lián)合常規(guī)治療,針刺穴位:睛明、太陽、百會、合谷、足三里;針對對照組患者則單純應用常規(guī)治療:局部應用糖皮質激素,靜脈滴注及皮下注射改善循環(huán)擴血管、營養(yǎng)神經藥,口服B族維生素,應用降眼壓眼水點眼,治療15天后觀察兩組治療前后患者的最佳矯正視力、視野的提高程度,評價臨床療效。結果:實驗組和對照組的視野平均缺損(MD)和最佳矯正視力在治療后與治療前相比,均具有統(tǒng)計學差異,P0.05;視野MD和最佳矯正視力分別在治療后進行兩組間對比,均具有統(tǒng)計學差異,P0.05;兩組間視野MD、最佳矯正視力在治療前后的差值對比,經統(tǒng)計學分析,均具有顯著差異,P0.01,說明經治療后針刺聯(lián)合常規(guī)治療組在視野平均缺損和最佳矯正視力的的改善均優(yōu)于常規(guī)治療組;針刺聯(lián)合常規(guī)治療組中患者治愈6例,顯效14例,有效11例,無效3例,總有效率91.17%,常規(guī)治療組中治愈2例,顯效7例,有效19例,無效6例,總有效率82.35%,經秩和檢驗,Z=2.594,P=0.009(P0.05),兩組療效差異有統(tǒng)計學意義,針刺聯(lián)合常規(guī)治療組療效優(yōu)于常規(guī)治療組。結論:針刺聯(lián)合常規(guī)療法治療前部缺血性視神經病變的臨床療效較單純應用常規(guī)治療要好,在常規(guī)治療的基礎上加以針刺,能夠更好的提升患者的最佳矯正視力,減輕視野平均缺損,促進患者視神經的恢復。
[Abstract]:Objective: to evaluate the efficacy of acupuncture combined with conventional therapy in the treatment of anterior ischemic optic neuropathy (AION). Materials and methods: sixty patients with anterior ischemic optic neuropathy were randomly divided into experimental group and control group. Zusanli; for the patients in the control group, only routine treatment was used: local administration of glucocorticoid, intravenous drip and subcutaneous injection to improve circulation and vasodilator, nutrition and neurodrugs, oral administration of group B vitamins, and eye drops of ocular pressure relief. After 15 days of treatment, the best corrected visual acuity and visual field were observed before and after treatment, and the clinical efficacy was evaluated. Results: the mean visual field defect (MD) and best corrected visual acuity (BCVA) in the experimental group and the control group were significantly different from those before treatment (P 0.05), the visual field MD and the best corrected visual acuity (BCVA) were compared between the two groups after treatment. There was statistical difference between the two groups (P 0.05). The difference between the two groups of MDS and BCVA before and after treatment was compared by statistical analysis. There were significant differences between the two groups (P 0.01), which indicated that the improvement of mean visual field defect and best corrected visual acuity in the acupuncture combined with routine treatment group was better than that in the conventional treatment group, and 6 cases were cured, 14 cases were effective and 11 cases were effective in the acupuncture combined with routine treatment group. In the routine treatment group, 2 cases were cured, 7 cases were markedly effective, 19 cases were effective, 6 cases were ineffective, and the total effective rate was 82.35%. The difference between the two groups was statistically significant (P0.05). The curative effect of acupuncture combined with routine treatment group was better than that of routine treatment group. Conclusion: the clinical effect of acupuncture combined with conventional therapy for anterior ischemic optic neuropathy is better than that of simple routine treatment. Acupuncture on the basis of routine treatment can better improve the best corrected visual acuity of patients. To reduce the mean defect of visual field and promote the recovery of optic nerve.
