掃頻光學(xué)相干斷層掃描在眼科的應(yīng)用進(jìn)展

【摘要】:精準(zhǔn)定性、定量評(píng)估眼前節(jié)及后節(jié)的影像學(xué)技術(shù),不僅對(duì)多種眼病的準(zhǔn)確診斷至關(guān)重要,同時(shí)也可為治療方案、手術(shù)方式以及隨訪提供重要參考依據(jù)。作為一種分辨率高、非接觸無(wú)創(chuàng)的成像技術(shù),光學(xué)相干斷層掃描(optical coherence tomography,OCT)已成為玻璃體視網(wǎng)膜相關(guān)疾病必不可少的輔助檢查工具。近幾年提出的掃頻光學(xué)相干斷層掃描成像(swept-source OCT,SS-OCT)技術(shù),具備掃描速度提高及穿透深度加深的優(yōu)點(diǎn),同時(shí)運(yùn)用不同成像系統(tǒng)可獲得眼部不同層次結(jié)構(gòu)精準(zhǔn)的成像。新型的SS-OCT成像技術(shù)可用于評(píng)估眼前節(jié)結(jié)構(gòu),如角膜移植術(shù)后角膜瓣、前房參數(shù)等檢查,也加深了對(duì)玻璃體視網(wǎng)膜相關(guān)疾病如糖尿病性黃斑病變、眼底新生血管的轉(zhuǎn)歸和黃斑毛細(xì)血管擴(kuò)張等疾病病理形態(tài)改變的認(rèn)識(shí)。同時(shí),運(yùn)用玻璃體增強(qiáng)成像(enhanced vitreous visualization,EVV)、分層掃描(En face SS-OCT)和SS-OCT血管造影等成像系統(tǒng),可為眼底相關(guān)疾病,如玻璃體后脫離、視網(wǎng)膜前膜、視網(wǎng)膜脫離、糖尿病牽拉性視網(wǎng)膜脫離等手術(shù)適應(yīng)證制定最佳的治療方案并協(xié)助術(shù)后隨訪。此外,該技術(shù)還為多種眼部疾病如青光眼、眼腫瘤的病理形態(tài)學(xué)觀察提供精準(zhǔn)的微結(jié)構(gòu)成像圖。本文主要對(duì)SS-OCT的技術(shù)原理及臨床應(yīng)用進(jìn)展等方面進(jìn)行綜述。
[Abstract]:Accurate qualitative and quantitative evaluation of the imaging techniques of anterior segment and posterior segment is not only very important for the accurate diagnosis of various eye diseases, but also provides an important reference for the treatment, operation and follow-up. As a high-resolution, non-contact non-invasive imaging technique, optical coherence tomography (Oct) has become an indispensable auxiliary examination tool for vitreoretinal diseases. In recent years, swept-source optical coherence tomography (SS-OCT) has the advantages of increasing scanning speed and deepening penetration depth. At the same time, different imaging systems can be used to obtain accurate imaging of different ocular layers. New SS-OCT imaging techniques can be used to evaluate the structure of anterior segment, such as corneal flap after corneal transplantation, parameters of anterior chamber and so on. It also deepens vitreoretinal diseases such as diabetic macular disease. Recognition of fundus neovascularization and macular capillary dilatation. At the same time, imaging systems such as vitreous enhancement imaging (enhanced vitreous visualization (EVV), layered scanning (En face SS-OCT) and SS-OCT angiography can be used as fundus related diseases, such as posterior vitreous detachment, preretinal membrane, retinal detachment, and so on. The optimal treatment for diabetic traction retinal detachment and other surgical indications was established and postoperative follow-up was assisted. In addition, the technique provides accurate microstructural imaging for various ocular diseases such as glaucoma and pathomorphology of eye tumors. This article reviews the technical principle and clinical application of SS-OCT.
