兒童人工耳蝸植入后聽(tīng)覺(jué)言語(yǔ)康復(fù)效果及個(gè)體影響因素分析

【摘要】:[目的]探討兒童人工耳蝸植入后不同階段的聽(tīng)覺(jué)言語(yǔ)康復(fù)效果;分析個(gè)體因素對(duì)人工耳蝸植入后效果的影響。[方法]根據(jù)植入年齡、耳聾基因檢測(cè)結(jié)果分別將130例確診為重度或極重度感音神經(jīng)性聾患兒分組,采用聽(tīng)覺(jué)行為分級(jí)標(biāo)準(zhǔn)(CAP)、言語(yǔ)可懂度分級(jí)標(biāo)準(zhǔn)(SIR)、嬰幼兒有意義聽(tīng)覺(jué)整合量表/有意義聽(tīng)覺(jué)整合量表(IT-MAIS/MAIS)、有意義言語(yǔ)使用量表(MUSS)對(duì)各組植入耳蝸患者0、3、6、12、18、24個(gè)月的聽(tīng)覺(jué)言語(yǔ)能力發(fā)展效果進(jìn)行評(píng)估比較。使用SPSS 19.0統(tǒng)計(jì)分析軟件和重復(fù)測(cè)量方差分析的統(tǒng)計(jì)方法,對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)。采用雙側(cè)檢驗(yàn),P0.05為差異有統(tǒng)計(jì)學(xué)意義。[結(jié)果]1~3歲組和3~5歲組患兒的預(yù)后效果明顯優(yōu)于其他年齡組患兒。GJB2基因相關(guān)性耳聾患者的術(shù)后效果明顯優(yōu)于其他組患者,而SLC26A4組患者雖然在表面上各量表得分高于未突變患者,但尚缺乏統(tǒng)計(jì)學(xué)依據(jù)。GJB3組患者的預(yù)后效果與未突變組患者預(yù)后情況基本一致。[結(jié)論]①聽(tīng)障兒童人工耳蝸植入后的聽(tīng)覺(jué)言語(yǔ)康復(fù)效果顯著;②植入年齡、耳聾基因是影響聽(tīng)覺(jué)言語(yǔ)康復(fù)效果的重要因素。
[Abstract]:[objective] to investigate the effects of auditory and verbal rehabilitation in different stages after cochlear implantation in children, and to analyze the influence of individual factors on the effect of cochlear implantation. [methods] 130 children with severe or extremely severe sensorineural hearing loss were divided into two groups according to the age of implantation and the results of gene test. Auditory behavior scale (CAP), speech intelligibility rating Standard (SIR), Infant meaningful auditory Integration scale / meaningful auditory Integration scale (IT-MAIS/MAIS); significant speech use scale (MUSS); The effect of language competence development is evaluated and compared. Using SPSS 19.0 statistical analysis software and the statistical method of repeated measurement ANOVA, the data were statistically analyzed. There was statistical significance in using bilateral test (P0.05). [results] 1 the prognostic effect of 3 years old group and 35 year old group was significantly better than that of other age group. GJB2 gene related deafness patients were better than other group. Although the scores of SLC26A4 patients were higher than those of non-mutation patients on the surface, the prognostic effect of GJB3 group was almost the same as that of non-mutation group. [conclusion] 1 the effect of hearing speech rehabilitation after cochlear implantation in children with hearing impairment is significant and the age of hearing loss gene is an important factor affecting the effect of hearing and speech rehabilitation.
