【摘要】：目的回顧性總結(jié)真菌性鼻竇炎的主要臨床特征,并探討真菌性鼻竇炎CT影像學特點。方法利用視覺模擬評分(VAS評分)、Lund-Kennedy評分、Lund-MacKay評分分別評價癥狀嚴重程度,內(nèi)鏡所見以及各竇腔CT圖像,統(tǒng)計首發(fā)癥狀和CT特征性的表現(xiàn)。對癥狀和評分進行Spearman相關(guān)分析。結(jié)果共有32例病患納入研究�；颊叩腖und-MacKay評分、Lund-Kennedy評分均與VSA評分呈正相關(guān)(P0.05)。患者的首發(fā)癥狀以頭痛、膿涕、鼻塞多見,涕血、面部疼痛、嗅覺下降、鼻腔異味不多見;鼻塞是最突出的癥狀(占46.9%)。鼻竇CT檢查發(fā)現(xiàn)鈣化灶伴軟組織影、骨質(zhì)增生及骨質(zhì)吸收/骨質(zhì)破壞病例數(shù)所占的比例較高,而單純軟組織影、氣液平等較少見。結(jié)論在真菌性鼻竇炎患者中,首發(fā)癥狀以頭痛、膿涕、鼻塞為主,而鼻塞在患者臨床表現(xiàn)中最為明顯;CT影像以鼻竇骨質(zhì)增生破壞并存為主。
[Abstract]:Objective to summarize the main clinical features of fungal sinusitis and to explore CT imaging features of fungal sinusitis. Methods Visual analogue score (VAS) and Lund-MacKay score were used to evaluate the severity of symptoms, endoscopic findings and CT images of each sinus cavity, and the initial symptoms and CT features were analyzed. The symptoms and scores were analyzed by Spearman. Results A total of 32 patients were included in the study. Lund-MacKay scores and Lund Kennedy scores were positively correlated with VSA scores (P0.05). The first symptoms of the patients were headache, purulent snot, nasal congestion, snot blood, facial pain, decreased olfaction, and nasal odour. Nasal congestion was the most prominent symptom (46.9%). Ct examination of paranasal sinuses showed that calcification with soft tissue shadow, bone hyperplasia and bone resorption / bone destruction accounted for a higher proportion, but simple soft tissue shadow, gas-liquid equality was rare. Conclusion in the patients with fungal sinusitis, the first symptoms were headache, purulent snot and nasal congestion.
