【摘要】：目的探討老年鼻息肉患者經(jīng)鼻內(nèi)鏡術(shù)切除后組織嗜酸粒細(xì)胞和腫瘤壞死因子(TNF)-α表達(dá)與預(yù)后的關(guān)系。方法回顧性分析行鼻內(nèi)鏡術(shù)切除老年鼻息肉患者40例,以手術(shù)6個(gè)月內(nèi)術(shù)腔完全上皮化和未完全上皮化的患者分為預(yù)后好組22例和預(yù)后差組18例。分別應(yīng)用免疫組織化學(xué)和蘇木精-伊紅染色的方法,研究兩組鼻息肉組織中嗜酸粒細(xì)胞和IFN-α的表達(dá),采用Logistic回歸分析法分析老年鼻息肉患者經(jīng)鼻內(nèi)鏡術(shù)切除后預(yù)后與組織嗜酸粒細(xì)胞與TNF-α的關(guān)系。結(jié)果預(yù)后好組鼻息肉組織中嗜酸粒細(xì)胞的表達(dá)較預(yù)后差組弱(P0.05);兩組鼻息肉組織中TNF-α的主要細(xì)胞有固有浸潤細(xì)胞、血管內(nèi)皮細(xì)胞、腺體細(xì)胞,均定位于細(xì)胞質(zhì)。在預(yù)后好組鼻息肉組織中,TNF-α的表達(dá)較預(yù)后差組弱(P0.05);有較多的炎癥細(xì)胞浸潤于鼻息肉組織的固有層,其中預(yù)后差組的嗜酸粒細(xì)胞TNF-α表達(dá)的總陽性率顯著高于預(yù)后好組(P0.01),Logistic回歸分析顯示,組織中嗜酸粒細(xì)胞與TNF-α是影響老年鼻息肉患者經(jīng)鼻內(nèi)鏡術(shù)切除后預(yù)后的重要因素(P0.05)。結(jié)論嗜酸粒細(xì)胞與TNF-α在鼻息肉組織中呈高表達(dá)狀態(tài),且影響老年鼻息肉鼻內(nèi)鏡切除術(shù)預(yù)后效果。
[Abstract]:Objective to investigate the relationship between the expression of eosinophil, tumor necrosis factor (TNF)-偽 and prognosis in elderly patients with nasal polyps after endoscopic resection. Methods 40 elderly patients with nasal polyps underwent endoscopic resection were retrospectively analyzed. The patients with complete epithelization and incomplete epithelization were divided into good prognosis group (22 cases) and poor prognosis group (18 cases). The expression of eosinophils and IFN- 偽 in nasal polyps was studied by immunohistochemistry and hematoxylin-eosin staining. The relationship between prognosis, tissue eosinophil and TNF- 偽 in elderly patients with nasal polyps was analyzed by Logistic regression analysis. Results the expression of eosinophil in nasal polyps with good prognosis was weaker than that in poor prognosis group (P0.05), and the main cells of TNF- 偽 in nasal polyp tissues were intrinsically infiltrated cells, vascular endothelial cells and glandular cells, which were located in cytoplasm. The expression of TNF- 偽 in nasal polyps with good prognosis was weaker than that with poor prognosis (P0.05), and more inflammatory cells infiltrated the lamina propria of nasal polyps. The positive rate of TNF- 偽 expression in the poor prognosis group was significantly higher than that in the good prognosis group (P0.01) logistic regression analysis showed that eosinophils and TNF- 偽 were the important factors influencing the prognosis of the elderly patients with nasal polyp after endoscopic resection (P0.05). Conclusion the expression of eosinophil and TNF- 偽 in nasal polyps is high, and it affects the prognosis of nasal endoscopic resection of nasal polyps in the elderly.
