【摘要】：背景和目的:視網膜分支動脈阻塞以及外傷性視神經病變均屬于眼科急癥。視網膜分支動脈阻塞臨床上較少見,常發(fā)生于60歲以上的中老年患者,發(fā)病因素多與動脈粥樣硬化、血管栓塞、動脈痙攣及炎性反應等相關。外傷性視神經病變多由頭面部外傷引起的直接性或間接性損傷導致。臨床工作中以及國內外文獻有關外傷導致視網膜分支動脈阻塞合并外傷性視神經病變的病例報道非常罕見。本文分享一例由外傷導致兒童視網膜分支動脈阻塞合并外傷性視神經病變的特殊病例,目的是探討這種復合眼外傷的發(fā)病機制、診斷標準、臨床特征和治療方法,為今后此類疾病的診治提供有價值的參考意見。方法:報告1例視網膜分支動脈阻塞合并外傷性視神經病變的病例。回顧性分析該患者的臨床資料,并查閱相關文獻綜合分析及討論。結論:外傷可作為視網膜分支動脈阻塞的獨立危險因素,根據患者的癥狀、體征、眼底熒光造影等不難確診。視網膜分支動脈阻塞合并外傷性視神經病變臨床上罕見,對視力威脅大。盡早明確診斷并積極治療,對挽救患者視力和改善視功能具有重要意義。
[Abstract]:Background and objective: retinal branch artery occlusion and traumatic optic neuropathy are ophthalmic emergencies. Retinal branch artery occlusion is rare in clinic and often occurs in the middle and old patients over 60 years old. The pathogenesis factors are mostly related to atherosclerosis, vascular embolism, arterial spasm and inflammatory reaction. Traumatic optic neuropathy is caused by direct or indirect injury caused by head and face trauma. It is rare to report cases of traumatic retinal artery occlusion associated with traumatic optic neuropathy in clinical work and literature at home and abroad. A special case of traumatic retinal branch artery occlusion associated with traumatic optic neuropathy was reported in this paper. The aim of this study was to investigate the pathogenesis, diagnostic criteria, clinical features and treatment of this compound ocular trauma. To provide valuable reference for the diagnosis and treatment of such diseases in the future. Methods: a case of retinal branch artery occlusion with traumatic optic neuropathy was reported. The clinical data of the patient were analyzed retrospectively, and the relevant literature were reviewed and discussed. Conclusion: trauma can be regarded as an independent risk factor of retinal branch artery occlusion. Retinal branch artery occlusion with traumatic optic neuropathy is rare clinically and is a great threat to visual acuity. Early diagnosis and active treatment are of great significance in saving visual acuity and improving visual function.
