【摘要】：目的:探討中鼻甲縫合技術(shù)對(duì)于功能性鼻內(nèi)鏡手術(shù)(FESS)術(shù)后中鼻甲外漂及中鼻道粘連閉塞的預(yù)防效果。方法:按EP3OS2007標(biāo)準(zhǔn)診斷為慢性鼻竇炎行FESS手術(shù)患者60例,隨機(jī)分為兩組,每組30例。A組患者在完成FESS術(shù)后行雙側(cè)中鼻道填塞,B組患者在FESS術(shù)后行雙側(cè)中鼻道填塞并雙側(cè)中鼻甲與鼻中隔縫合。術(shù)后半年復(fù)查鼻內(nèi)鏡及鼻竇CT,兩組各失訪1例,資料完整者58例,對(duì)比兩組患者發(fā)生中鼻甲外漂、中鼻道粘連及鼻竇炎復(fù)發(fā)的情況。結(jié)果:兩組各29例(58側(cè)),術(shù)后中鼻甲外漂A組10側(cè),B組2側(cè),兩組差異有顯著性(P0.05);中鼻道粘連A組8側(cè),B組2側(cè),兩組差異有顯著性(P0.05);術(shù)后復(fù)查鼻竇CT發(fā)現(xiàn)鼻竇炎復(fù)發(fā)或殘留A組13側(cè),B組4側(cè),兩組差異有顯著性(P0.05)。結(jié)論:中鼻甲與鼻中隔縫合技術(shù)簡(jiǎn)單、實(shí)用,利于FESS術(shù)后保持中鼻甲位置穩(wěn)定性、形成持久開放的中鼻道及預(yù)防鼻竇炎復(fù)發(fā)。
[Abstract]:Objective: to investigate the prevention effect of middle turbinate suture technique on middle turbinate bleach and middle nasal canal adhesion occlusion after functional endoscopic sinus surgery (FESS). Methods: according to EP3OS2007 criteria, 60 patients with chronic sinusitis undergoing FESS operation were randomly divided into two groups. 30 patients in group A underwent bilateral middle nasal canal tamponade after FESS. Group B underwent bilateral middle nasal canal tamponade and bilateral middle turbinate and nasal septum suture after FESS. Half a year after operation, nasal endoscopy and sinus CTS were reexamined. One case was lost in each group, 58 cases had complete data. The incidence of middle turbinate bleach, middle nasal canal adhesion and recurrence of sinusitis were compared between the two groups. Results: there were 29 cases (58 sides) in each group, 2 sides in group B, 10 sides in group A, 2 sides in group B (P0.05), 2 sides in group A, 2 sides in group B, 8 sides in group A, 2 sides in group B, 2 sides in group A and 2 sides in group B, respectively. There was a significant difference between the two groups (P0.05); after reexamination of sinus CT found recurrent or residual sinusitis in group A 13 sides of B group 4 sides, there was a significant difference between the two groups (P0.05). Conclusion: the technique of suture between middle turbinate and nasal septum is simple and practical, which is helpful to maintain the stability of middle turbinate after FESS, to form a permanent open middle nasal canal and to prevent the recurrence of sinusitis.
