基于角膜共焦激光顯微鏡檢測(cè)的角膜基底神經(jīng)變化與2型糖尿病視網(wǎng)膜微血管病變的相關(guān)性研究

【摘要】:目的探討角膜共焦激光顯微鏡檢測(cè)的角膜基底神經(jīng)變化與2型糖尿病(type 2 diabetes mellitus,T2DM)視網(wǎng)膜微血管病變的相關(guān)性。方法選取2016年2月至2017年2月在我院治療的T2DM患者118例,其中合并糖尿病視網(wǎng)膜病變(diabetic retinopathy,DR)患者57例(DR組),無(wú)DR患者61例(NDR組),同時(shí)選取健康志愿者60人作為對(duì)照組,所有研究對(duì)象采用角膜共焦激光顯微鏡檢查,分析角膜神經(jīng)形態(tài)參數(shù)與臨床指標(biāo)的關(guān)系。結(jié)果 DR組角膜神經(jīng)纖維密度、角膜神經(jīng)分支密度和角膜神經(jīng)長(zhǎng)度分別為(20.03±4.22)條·mm-2、(22.01±7.05)條·mm-2和(9.50±1.76)mm·mm-2,明顯小于對(duì)照組和NDR組(均為P0.05),而角膜神經(jīng)纖維彎曲度為0.30±0.03,明顯高于對(duì)照組和NDR組(均為P0.05);DR患者中,Ⅲ期患者角膜神經(jīng)纖維密度、角膜神經(jīng)分支密度和角膜神經(jīng)長(zhǎng)度明顯小于Ⅰ期和Ⅱ期DR患者(均為P0.05),而角膜神經(jīng)纖維彎曲度明顯高于Ⅰ期和Ⅱ期DR患者(均為P0.05)。DR組病程為(12.04±2.48)a,明顯高于NDR組(P0.05),而空腹C肽和空腹胰島素分別為(1.41±0.58)μg·L-1和(20.05±7.91)m U·L-1,明顯低于NDR組(均為P0.05)。T2DM患者病程與角膜神經(jīng)分支密度及角膜神經(jīng)纖維長(zhǎng)度呈負(fù)相關(guān)(r=-0.322、-0.317,均為P0.05);空腹C肽與角膜神經(jīng)分支密度呈正相關(guān)(r=0.298,P0.05),與角膜神經(jīng)彎曲度呈負(fù)相關(guān)(r=-0.311,P0.05)。結(jié)論 T2DM視網(wǎng)膜微血管病變患者角膜神經(jīng)形態(tài)參數(shù)異常,對(duì)病程較長(zhǎng)或空腹C肽水平低的T2DM患者應(yīng)用角膜共激光顯微鏡檢測(cè)有助于早期發(fā)現(xiàn)微血管病變。
[Abstract]:Objective to investigate the correlation between corneal basal nerve changes and type 2 diabetes mellitus (type 2 diabetes mellitus T2DM) by confocal laser microscopy.
