計算機流體力學在模擬下鼻甲手術(shù)中的應用

【摘要】:目的:建立模擬兩種下鼻甲切除術(shù)式的計算機流體力學即CFD模型,分析其對鼻腔流體力學的影響。方法:建立單側(cè)下鼻甲肥大鼻腔的CFD模型A,在此基礎(chǔ)上模擬下鼻甲切除術(shù)分別建立B、C兩種術(shù)后模型,運用流體分析軟件Fluent6.3.26計算三種模型的鼻腔全流場數(shù)據(jù)。結(jié)果:B模型患側(cè)鼻腔面積較原始模型平均面積增大0.36cm2,C模型增大0.89cm2;B模型患側(cè)鼻腔的壓降差(約8Pa)與原始模型相比無太大改變,C模型壓降差明顯降低(約3Pa)。B模型患側(cè)鼻腔流量無明顯增加,C模型總鼻道下方及下鼻道流量為80ml/s,約為原始模型的8倍。B模型在吸氣相及呼氣相氣流流速及流動方式與原始模型無明顯改變,C模型在鼻瓣區(qū)流速明顯增加,達到1.04m/s,并且渦流的產(chǎn)生與正常鼻腔趨于一致。結(jié)論:模型C在恢復鼻腔的正常解剖形態(tài)及正常通氣生理方面都明顯優(yōu)于模型B。在下鼻甲手術(shù)中,恢復鼻腔的正常解剖結(jié)構(gòu)對于鼻腔疾病的治療具有決定性的意義。
[Abstract]:Aim: to establish a computer fluid dynamics (CFD) model, which simulates two types of inferior turbinate resection, and to analyze its effect on nasal fluid dynamics. Methods: a CFD model of unilateral inferior turbinate hypertrophy nasal cavity was established. On this basis, two kinds of BPC models were established by simulating inferior turbinate resection. The flow field data of the three models were calculated by fluid analysis software Fluent6.3.26. Results compared with the original model, the nasal cavity area of the affected side of the ratio B model increased by 0.36 cm ~ (2) and C model increased by 0.89 cm ~ (2) C model. The pressure drop difference (about 8 Pa) of the affected side of the B model had no significant change compared with the original model. The pressure drop difference of the C model (about 3 Pa) .B model was significantly lower than that of the original model (about 3 Pa). There was no significant increase in nasal cavity flow under the total nasal tract and the lower nasal canal in the C model, which was about 8 times of that of the original model. The flow rate and flow pattern in the inspiratory and expiratory phases of the model were not significantly different from those of the original model. The velocity of the area increased obviously, Up to 1. 04 m / s, and eddy current production tends to be consistent with the normal nasal cavity. Conclusion: model C is superior to model B in restoring normal anatomic morphology and normal ventilation physiology of nasal cavity. In inferior turbinate surgery, the restoration of normal anatomic structure of nasal cavity plays a decisive role in the treatment of nasal diseases.
