RASSF1A基因在喉鱗狀細胞癌及喉角化癥組織中的表達及其臨床意義

【摘要】:目的 喉鱗狀細胞癌是指發(fā)生于喉黏膜上皮的惡性腫瘤,是耳鼻咽喉頭頸外科最常見的惡性腫瘤之一。喉癌抑癌基因RASSF1A的異常表達與喉鱗狀細胞癌的關(guān)系近年來國內(nèi)鮮有報道。RASSF1A基因通過干預(yù)細胞周期、抑制細胞生長、促進細胞凋亡而抑制腫瘤的發(fā)生。該基因啟動子發(fā)生甲基化之后會引起基因的失活,導(dǎo)致細胞周期紊亂而發(fā)生癌變。本研究觀察喉鱗狀細胞癌組織中的RASSF1A表達情況,探討RASSF1A基因的表達與喉鱗狀細胞癌的發(fā)生及病變進展的關(guān)系,從而有望在臨床診療中使其成為一項新的診斷及預(yù)后評估的指標(biāo),以提高早期診斷率及腫瘤的治愈率。 喉角化癥是喉的癌前病變,是喉部黏膜上皮生長、成熟異常以及黏膜上皮過度角化的一種增生性疾病。RASSF1A基因在喉的癌前病變組織中的甲基化及表達方面研究甚少。本研究觀察喉角化癥組織中RASSF1A的表達情況,來初步探討RASSF1A基因與喉角化癥發(fā)病是否具有相關(guān)性。 方法 收集安徽省立耳鼻咽喉頭頸外科自2009-2011年的術(shù)中切除的聲帶息肉、喉角化癥及喉鱗狀細胞癌組織,另外取尸頭正常聲帶黏膜。用免疫組織化學(xué)方法檢測各組組織的RASSF1A蛋白表達情況。RASSF1A蛋白經(jīng)免疫組織化學(xué)方法染色后為棕黃色顆粒,在細胞漿內(nèi)彌漫性分布,細胞核不著色。計算每例組織切片中RASSF1A陽性細胞百分?jǐn)?shù),取平均值±標(biāo)準(zhǔn)差(x±s),采用SPSS17.0統(tǒng)計分析軟件對數(shù)據(jù)進行分析,P<0.05為差異有統(tǒng)計學(xué)意義。 結(jié)果 RASSF1A在喉鱗狀細胞癌及喉角化癥組織中的表達低于聲帶息肉組織和正常喉黏膜組織;喉鱗狀細胞癌與聲帶息肉,喉鱗狀細胞癌與正常喉黏膜及喉鱗狀細胞癌與喉角化癥進行比較,RASSF1A表達差異均具有統(tǒng)計學(xué)意義(P0.05)。 喉角化癥與聲帶息肉比較,喉角化癥與正常喉黏膜組織比較以及喉角化癥各類型之間進行比較,RASSF1A表達差異不具有統(tǒng)計學(xué)意義(P0.05)。 RASSF1A在喉鱗狀細胞癌臨床I-IV期中的表達情況呈現(xiàn)逐漸下降趨勢,各臨床分期間差異具有統(tǒng)計學(xué)意義(P0.05)。RASSF1A在高分化、中分化及低分化癌組織中表達差異以及各年齡組之間表達的差異亦不具有統(tǒng)計學(xué)意(P0.05)。 結(jié)論 1.喉鱗狀細胞癌組織中RASSFlA蛋白的表達程度明顯低于正常喉組織及聲帶息肉組織,提示RASSFlA蛋白表達降低與喉鱗狀細胞癌的發(fā)生有關(guān); 2.RASSFlA的表達與患者年齡及組織的病理學(xué)分化程度無關(guān),但與喉鱗狀細胞癌的臨床分期有關(guān),不同臨床分期喉癌組織RASSFlA蛋白的表達程度不同,臨床分期越早,RASSFlA蛋白表達水平越高;臨床分期越晚,RASSFlA蛋白表達水平越低;提示RASSFlA的表達可能與腫瘤的浸潤及轉(zhuǎn)移有關(guān)。 3.RASSF1A基因可能參與了喉鱗狀細胞癌發(fā)生的早期過程,因此可以利用此特征作為喉鱗狀細胞癌的早期診斷指標(biāo),并且對于在鑒別癌前病變與喉癌時可能具有一定意義; 4.對RASSFlA蛋白表達進行檢測,有助于篩查RASSFlA基因異常的人群,,提搞喉鱗狀細胞癌的早期診斷水平。
[Abstract]:Purpose

The expression of RASSF1A gene in laryngeal squamous cell carcinoma and its relationship with laryngeal squamous cell carcinoma have been reported .

RASSF1A gene is a kind of proliferative disease in laryngeal carcinoma . The RASSF1A gene has been used to investigate the methylation and expression of RASSF1A gene in laryngeal carcinoma .

method

The expression of RASSF1A protein was detected by immunohistochemical method . RASSF1A protein was stained by immunohistochemistry . The percentage of RASSF1A positive cells in each group was calculated and the mean 鹵 standard deviation ( x 鹵 s ) was calculated . The data were analyzed by SPSS 17.0 statistical analysis software , and the difference was statistically significant .

Results

The expression of RASSF1A in laryngeal squamous cell carcinoma and laryngeal keratopathy was lower than that of vocal cord polyp and normal laryngeal mucosa .
The expression of RASSF1A in laryngeal squamous cell carcinoma and vocal cord polyp , laryngeal squamous cell carcinoma and normal laryngeal mucosa and laryngeal squamous cell carcinoma were statistically significant ( P0.05 ) .

There was no significant difference in RASSF1A expression compared with vocal cord polyp ( P0.05 ) .

RASSF1A showed a decreasing trend in the clinical I - IV period of laryngeal squamous cell carcinoma ( P0.05 ) . RASSF1A had no statistical significance in the expression of RASSF1A in high differentiation , middle differentiation and low differentiated cancer tissues and the difference between different age groups ( P0.05 ) .

Conclusion

1 . The expression of RASSFlA protein in laryngeal squamous cell carcinoma was significantly lower than that in normal laryngeal tissues and vocal cord polyp tissues , suggesting that RASSFlA protein expression was associated with the occurrence of laryngeal squamous cell carcinoma ;

2 . The expression of RASSFlA was not related to the age of the patient and the pathological differentiation degree of the tissues , but the expression level of RASSFlA protein was different from the clinical stage of laryngeal squamous cell carcinoma . The higher the expression level of RASSFlA protein in different clinical stages , the higher the expression level of RASSFlA protein , the lower the level of RASSFlA protein expression was .
It is suggested that the expression of RASSFlA may be related to invasion and metastasis of tumor .

3 . RASSF1A gene may be involved in the early stage of laryngeal squamous cell carcinoma , so it can be used as an early diagnostic index for laryngeal squamous cell carcinoma , and may be of certain significance in identifying precancerous lesions and laryngeal cancer ;

4 . The expression of RASSFlA protein is detected , which helps to screen the abnormal population of RASSFlA gene , which can be used for early diagnosis of laryngeal squamous cell carcinoma .
