阻塞性睡眠呼吸暫停低通氣綜合征合并糖尿病患者血清NO和維他命E含量水平的變化

【摘要】:目的:本文通過觀察研究對象血清中一氧化氮(NO)和維他命E(VE)水平數(shù)值的變化,探討NO、VE在阻塞性睡眠呼吸暫停低通氣綜合征并發(fā)2型糖尿發(fā)生發(fā)展過程中的作用。方法:收集2015年1月-2017年2月于山西醫(yī)科大學(xué)第一醫(yī)院呼吸睡眠監(jiān)測室、內(nèi)分泌科室檢查及住院的病人群體,按照相關(guān)診斷標(biāo)準(zhǔn)分為單純OSAHS組(OA組),OSAHS合并糖尿病組(OD組),糖尿病組(DM組),正常組(NC組)四組,共計65例,均行多導(dǎo)睡眠監(jiān)測(PSG),分別記錄患者的性別、年齡,測量身高、重量,計算出體重指數(shù)(BMI),手動修正睡眠監(jiān)測報告,并記錄每人呼吸暫停指數(shù)(AHI)、夜間最低血氧飽和度(LSO2),且于當(dāng)日行睡眠監(jiān)測之前抽取肘靜脈血,離心后留取血清,采用比色法和微板法分別測定各組血清中VE和NO含量水平,并對四組相關(guān)數(shù)據(jù)進行整理,統(tǒng)計學(xué)描寫和SPSS軟件數(shù)據(jù)資料分析。結(jié)果:1.一般特征比較:四組之間年齡、BMI不存在統(tǒng)計學(xué)差別(P0.05)。2.呼吸參數(shù)的比較:OA組、OD組兩組的AHI、LSO2分別與NC組、DM組存在統(tǒng)計學(xué)差別(P值均0.05)。3.血清NO、VE水平測定的結(jié)果:與NC組比較,其余三組的VE、NO數(shù)值均下降,OD組下降尤為顯著,四組間差別存在統(tǒng)計學(xué)意義(P0.05)。4.Person相關(guān)分析:VE水平數(shù)值與AHI呈負相關(guān),與LSO2呈正相關(guān)。NO水平數(shù)值與AHI呈負相關(guān),與LSO2呈正相關(guān)。結(jié)論:1.與正常健康人群相比,OSAHS患者NO、VE水平下降,病情愈嚴(yán)重,二者水平下降愈明顯,且與AHI成負相關(guān),與LSO2呈正相關(guān)。2.糖尿病病人NO、VE較正常健康人群下降,體內(nèi)存在一定水平的氧化應(yīng)激。3.OSAHS合并糖尿病組較單一病種組NO、VE水平下降顯著,氧化應(yīng)激水平增強,引發(fā)IR,可能是OSAHS合并糖尿病的發(fā)生機制之一。
[Abstract]:Objective: to investigate the role of nitric oxide (no) and vitamin E (VE) in the pathogenesis of type 2 diabetes in obstructive sleep apnea hypopnea syndrome (OSAS) by observing the changes of serum nitric oxide (no) and vitamin E (VE) levels. Methods: the patients were collected from January 2015 to February 2017 in the respiratory and sleep monitoring room of the first Hospital of Shanxi Medical University, the Endocrinology Department and the inpatients. According to the diagnostic criteria, 65 patients were divided into four groups: OSAHS group (OD group), diabetes group (DM group), and normal group (NC group). The patients were monitored by polysomnography (PSG), and their sex, age, height and weight were measured. Body mass index (BMI) was calculated, sleep monitoring report was corrected manually, apnea index (AHI) and minimum oxygen saturation (LSO2) were recorded. The levels of VE and no in serum of each group were measured by colorimetric method and microplate method respectively. The related data of four groups were sorted out statistical description and data analysis of SPSS software were carried out. The result is 1: 1. Comparison of general characteristics: there was no statistical difference in BMI between the four groups (P0.05). 2. Comparison of respiratory parameters in the OD group and the control group, there were significant differences between the two groups (P < 0.05). Results: compared with NC group, the values of VENO in the other three groups decreased significantly, and the differences among the four groups were statistically significant (P0.05) .4.Person correlation analysis showed that there was a negative correlation between VENO and AHI in the other three groups. The level of no was negatively correlated with AHI and positively correlated with LSO2. Conclusion 1. Compared with the healthy people, the level of NOVE in OSAHS patients decreased, the more serious the disease, the more obvious the decrease of the two levels, and the negative correlation with AHI, and the positive correlation with LSO2. 2. The level of NOTVE in diabetic patients was significantly lower than that in normal controls. There was a certain level of oxidative stress in the body. 3. The level of NOFVE in OSAHS with diabetes mellitus group was significantly lower than that in the single disease group, and the level of oxidative stress was increased. The initiation of IRI may be one of the mechanisms of OSAHS with diabetes.
