【摘要】：目的:觀察釔鋁石榴石激光淚道成形術(shù)(Nd:YAG激光淚道成形術(shù))中應用妥布霉素地塞米松眼膏的臨床療效及安全性。方法:選取我院2013年1月-2017年1月在Nd:YAG激光淚道成形術(shù)中應用妥布霉素地塞米松眼膏治療的阻塞性淚道疾病患者1 000例(1 250眼)的病歷資料,按淚道阻塞的部位分為慢性淚囊炎組(380眼)、淚小(總)管阻塞組(360眼)、鼻淚管阻塞組(342眼)、淚小點阻塞組(86眼)及外傷性淚小管離斷組(82眼)。5組患眼術(shù)中將稍加熱后的妥布霉素地塞米松眼膏裝入1 m L注射器內(nèi)通過探針注入淚道內(nèi),邊推注邊退出探針。術(shù)后定期沖洗淚道,隨訪3個月。對各組療效及安全性進行比較。結(jié)果:慢性淚囊炎組患眼術(shù)后治愈率為85.00%、有效率為87.63%,淚小(總)管阻塞組患眼術(shù)后治愈率為87.78%、有效率為94.45%,鼻淚管阻塞組患眼術(shù)后治愈率為89.18%、有效率為95.03%,淚小點阻塞組患眼術(shù)后治愈率為84.88%、有效率為94.18%,均顯著高于外傷性淚小管離斷組患眼的治愈率(42.68%)、有效率(64.63%),差異均有統(tǒng)計學意義(P0.05)。5組患眼不良反應發(fā)生率比較,差異均無統(tǒng)計學意義(P0.05)。結(jié)論:Nd:YAG激光淚道成形術(shù)在治療慢性淚囊炎、淚小(總)管阻塞、鼻淚管阻塞、淚小點阻塞中應用妥布霉素地塞米松眼膏的療效較好,且不良反應發(fā)生率較低。
[Abstract]:Objective: to observe the clinical efficacy and safety of tobramycin dexamethasone eye ointment in ND: YAG laser lacrimal duct angioplasty. Methods: the medical records of 1 000 patients (1 250 eyes) with obstructive lacrimal duct disease treated with tobramycin dexamethasone eye ointment in ND: YAG laser lacrimal duct angioplasty from January 2013 to January 2017 were selected. According to the location of lacrimal duct obstruction, the patients were divided into three groups: chronic dacryocystitis group (380 eyes), small lacrimal duct occlusion group (360 eyes), nasolacrimal duct obstruction group (342 eyes), lacrimal dot occlusion group (86 eyes) and traumatic lacrimal canaliculus disconnection group (82 eyes). Tobramycin dexamethasone eye ointment was injected into the lacrimal duct with a probe in a 1 mL syringe. Push and exit the probe. Lacrimal duct was flushed regularly after operation and followed up for 3 months. The efficacy and safety of each group were compared. Results: in the chronic dacryocystitis group, the postoperative cure rate was 85.00, the effective rate was 87.63, the postoperative cure rate was 87.78 and 94.455.The nasolacrimal duct obstruction group was 89.18, and the effective rate was 95.03. The cure rate was 84.88 and the effective rate was 94.18 in the plug group, which was significantly higher than that in the traumatic lacrimal canaliculus disconnection group (42.68%) and the effective rate (64.63%). The difference was statistically significant (P0.05). The difference was not statistically significant (P0.05). Conclusion the application of tobramycin dexamethasone ointment in the treatment of chronic dacryocystitis, obstruction of lacrimal (total) duct, obstruction of nasolacrimal duct and lacrimal dot is effective in the treatment of chronic dacryocystitis, and the incidence of adverse reactions is low.
