鼻咽癌裸鼠移植瘤ADC值的早期變化與輻射敏感性的相關(guān)研究

【摘要】:摘要:目的:利用MRI擴(kuò)散加權(quán)成像(DW-MRI)探討不同輻射敏感性移植瘤的ADC值的變化與照射相關(guān)體積變化之間的關(guān)系,以預(yù)測(cè)荷人鼻咽癌裸鼠移植瘤的輻射敏感性。材料與方法:利用輻射敏感性不同的CNE1和CNE2細(xì)胞株建立荷人鼻咽癌移植瘤裸鼠共分為4組,兩系照射組和對(duì)照組各10只,成瘤后隔天測(cè)量腫瘤大小,照射組CNE1t、CNE2t行MRI擴(kuò)散加權(quán)成像掃描,并測(cè)平均ADC值(ADC_0),15-20小時(shí)后用6MVX單次照射15Gy,再次行DW-MRI掃描對(duì)照組CNE1c、CNE2c不照射,測(cè)量時(shí)間與照射組相同。結(jié)果:CNE1c組裸鼠因麻醉意外死亡一只。CNE1c、CNE1t、CNE2c、CNE2t的ADC_0、ADC_1值分別為(1.708±0.458)×10~(-3)mm~2/s、(1.684±0.441)×10~(-3)mm~2/s,(1.705±0.522)×10~(-3)mm~2/s、(1.603±0.250)×10~(-3)mm~2/s,(1.297±0.623)×10~(-3)mm~2/s、(1.049±0.260)×10~(-3)mm~2/s和(1.647±0.312)×10~(-3)mm~2/s、(0.847±0.306)×10~(-3)mm~2/s,各組內(nèi)ADC_0、ADC_1值變化,僅CNE2t組ADC_1值較ADC_0值顯著減。≒㩳0.001),其余三組無顯著變化(P值分別為0.699,0.575,0.212)。CNE1c組和CNE1t組ADC_(1-0)、Δ ADC分別為(-0.240±1.902)×10-4mm~2/s (-1.017±5.215)×10-4mm~2/s、(-0.727±12.175)%(1.221±32.074)%,兩組相比ADC_0、ADC_1、ADC_(1-0)、ΔADC均無統(tǒng)計(jì)學(xué)差異(P值分別為0.989,0.635,0.682,0.867);而CNE2c組和CNE2t組ADC_(1-0)、ΔADC分別為(-2.480±5.844)×10-4mm~2/(s-7.994±3.827)×10-4mm~2/s、(-6.527±43.944)%(-46.739±22.816)%,兩組相比,ADC_0、ADC_1無統(tǒng)計(jì)學(xué)差異(P值均為0.130),而ADC_(1-0)、ΔADC差異顯著(P值為0.022,0.019)。兩對(duì)照組相比,ADC_1的統(tǒng)計(jì)學(xué)差異顯著(t=3.924,P=0.001),ADC_0、ADC_(1-0)、ΔADC無統(tǒng)計(jì)學(xué)差異(P值分別為0.110、0.264、0.696);但兩照射組除ADC_0(t=0.300,P=0.768)外,ADC_1、ADC_(1-0)、ΔADC差異顯著(P值分別為㩳0.001、0.004、0.001)。從照射后第4天開始,兩照射組δVx差異顯著(P值均㩳0.001)。從照射后第2天開始,δVx與ΔADC之間存在負(fù)相關(guān)關(guān)系,,從照射后第4天開始,δVx與細(xì)胞株類型正相關(guān)。采用多元線性回歸分析,從第14天開始,細(xì)胞株類型及ΔADC為δVx的影響因素。結(jié)論:不同輻射敏感性的移植瘤照射后ADC值變化顯著不同;照射前后ADC值變化程度是照射相關(guān)移植瘤體積變化的影響因素。因此認(rèn)為照射后ADC值的早期變化能夠預(yù)測(cè)鼻咽癌輻射敏感性,照射早期ADC值下降幅度越大其輻射敏感性越高。
[Abstract]:Abstract: objective: to study the relationship between the change of ADC value and the change of irradiation-related volume in xenografts with different radiosensitivity by using diffusion-weighted MRI (DW-MRI) to predict the radiosensitivity of xenografts bearing human nasopharyngeal carcinoma (NPC) in nude mice. Materials and methods: nude mice bearing human nasopharyngeal carcinoma transplanted tumor were established by using CNE1 and CNE2 cell lines with different radiosensitivity. Nude mice were divided into 4 groups. The patients in the irradiation group underwent diffusion-weighted MRI imaging, and the average ADC value was measured 15 to 20 hours after exposure to 6MVX at a single dose of 15 Gy.
