【摘要】：目的探討在兒童感染性鼻炎中運用生理性海水鼻腔噴霧器(鼻郎)加糜蛋白酶治療的臨床效果,旨在為今后臨床治療兒童感染性鼻炎提供參考與借鑒。方法前瞻性選取2013年1月-2017年1月醫(yī)院兒科收治的98例患有感染性鼻炎兒童為研究對象,采用信封法分為試驗組和對照組,每組各49例;對照組采用常規(guī)抗感染治療,試驗組在對照組的基礎(chǔ)上,在生理性海水鼻腔噴霧器(鼻郎)中加入注射用糜蛋白酶4 000u配制成混合制劑進(jìn)行噴鼻治療;采用視覺模擬量表(Visual analogue scal,VAS)及Lund-kennedy評分評估感染性鼻炎患兒治療前后的臨床治療效果;通過酶聯(lián)免疫法監(jiān)測兩組治療前后白細(xì)胞介素-17(IL-17)、IL-17受體(IL-17R)水平及免疫球蛋白IgE含量。結(jié)果試驗組患兒總有效率95.92%,高于對照組81.63%(P0.05);治療4周后,兩組各項癥狀評分均有不同程度改善,且試驗組改善程度優(yōu)于對照組(P0.05);治療2周后,兩組血清中炎癥介質(zhì)IL-17、IL-17R水平及免疫球蛋白IgE含量均較治療前有改善,且試驗組改善程度優(yōu)于對照組(P0.05)。結(jié)論將生理性海水鼻腔噴霧器(鼻郎)加糜蛋白酶運用于兒童感染性鼻炎治療,能夠迅速緩解患兒鼻部臨床癥狀,具有高效、安全性能高、使用簡便等優(yōu)勢。
[Abstract]:Objective to explore the clinical effect of physiologic seawater nasal spray plus chymotrypsin in the treatment of infective rhinitis in children in order to provide reference for clinical treatment of infective rhinitis in children. Methods 98 pediatric children with infectious rhinitis were prospectively selected from January 2013 to January 2017. They were divided into experimental group (n = 49) and control group (n = 49). On the basis of the control group, the experimental group added chymotrypsin 4000 u for injection into the physiologic sea water nasal spray (rhinorang) for nasal spray treatment. Visual analogue scalar vas and Lund-kennedy scores were used to evaluate the clinical therapeutic effect of children with infectious rhinitis before and after treatment, and the levels of IL-17 IL-17 receptor IL-17R and the content of immunoglobulin IgE were measured by enzyme-linked immunosorbent assay (Elisa). Results the total effective rate of the children in the test group was 95.92 and was higher than that in the control group (81.63 and P 0.05). After 4 weeks of treatment, the scores of symptoms in the two groups were improved to some extent, and the improvement degree of the test group was better than that of the control group (P 0.05), and after 2 weeks of treatment, the improvement was better than that of the control group. The levels of IL-17, IL-17R and immunoglobulin IgE in serum of the two groups were improved compared with those before treatment, and the improvement degree of the test group was better than that of the control group (P 0.05). Conclusion the application of physiologic sea water nasal sprayer plus chymotrypsin in the treatment of infective rhinitis in children can relieve the clinical symptoms of infective rhinitis of children quickly, and has the advantages of high efficiency, high safety performance and easy to use.
