【摘要】：目的探討老年阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患者血清髓過(guò)氧化物酶(MPO)、細(xì)胞間黏附分子(ICAM)-1的表達(dá)及意義。方法通過(guò)多導(dǎo)睡眠監(jiān)測(cè)儀(PSG)確診的老年OSAHS患者93例為OSAHS組,根據(jù)睡眠呼吸暫停低通氣指數(shù)(AHI)分為重、中、輕3組;選取31例健康老年體檢者為對(duì)照組;酶聯(lián)免疫吸附法(ELISA)測(cè)定入組者外周血ICAM-1、MPO濃度;重度OSAHS患者進(jìn)行綜合治療3個(gè)月后復(fù)測(cè)ICAM-1、MPO,分析ICAM-1、MPO與PSG指標(biāo)的相關(guān)性。結(jié)果老年OSAHS患者ICAM-1、MPO濃度隨病情嚴(yán)重程度增加逐漸上升(F=29.937,18.946;P0.01);輕度組ICAM-1、MPO濃度與對(duì)照組無(wú)顯著差異(P0.05);余各組相互比較均存在顯著差異(P0.05)。OSAHS患者ICAM-1、MPO水平與體重指數(shù)(BMI)、年齡無(wú)明顯關(guān)聯(lián)(P0.05);OSAHS患者ICAM-1與MPO水平呈正相關(guān),二者均和夜間最低血氧飽和度(Lsa O2)呈負(fù)相關(guān),與AHI呈正相關(guān)(P0.01)。重度OSAHS患者經(jīng)3個(gè)月綜合治療后外周血ICAM-1、MPO濃度及AHI顯著降低,Lsa O2明顯上升,和治療前比較均有顯著差異(P0.01)。結(jié)論老年OSAHS患者體內(nèi)ICAM-1、MPO水平上升可能為其并發(fā)心血管損害的機(jī)制之一;外周血ICAM-1、MPO水平的測(cè)定對(duì)老年OSAHS病情判斷、療效評(píng)估及合并心血管病風(fēng)險(xiǎn)的預(yù)測(cè)有一定參考價(jià)值。
[Abstract]:Objective to investigate the expression and significance of serum myeloperoxidase (MPO) and intercellular adhesion molecule (ICAM-1) in elderly patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods 93 elderly OSAHS patients diagnosed by PSG were divided into 3 groups according to sleep apnea hypopnea index (AHI), 31 healthy old people as control group. The concentration of ICAM-1 MPO in peripheral blood of patients with severe OSAHS was determined by Elisa, and the correlation between ICAM-1 MPO and PSG index was analyzed after 3 months of comprehensive treatment in patients with severe OSAHS. Results the concentration of ICAM-1 MPO in elderly patients with OSAHS increased gradually with the increase of severity of the disease. The concentration of ICAM-1 MPO in the mild group was not significantly different from that in the control group (P0.05), and there was significant difference between the other groups in the level of ICAM-1MPO and the body mass index (BMIA) in the patients with OSAHS. There was a positive correlation between ICAM-1 and MPO level in patients with OSAHS. Both of them were negatively correlated with the nocturnal minimum oxygen saturation (LSAO _ 2) and positively correlated with AHI (P 0.01). After 3 months of comprehensive treatment, the concentration of ICAM-1MPO and AHI in patients with severe OSAHS significantly decreased and LsaO2 increased significantly, and there was significant difference between them before treatment and before treatment (P 0.01). Conclusion the increase of ICAM-1 MPO level in elderly patients with OSAHS may be one of the mechanisms of cardiovascular damage, and the determination of ICAM-1 MPO level in peripheral blood may be of some reference value in judging the condition of OSAHS, evaluating the curative effect and predicting the risk of cardiovascular disease in the elderly patients with OSAHS.
