【摘要】：目的:針對(duì)確診的聲帶突肉芽腫,進(jìn)行質(zhì)子泵抑制劑(PPI)聯(lián)合胃動(dòng)力藥物治療與支撐喉鏡下肉毒素注射治療的對(duì)照研究,分析比較2種方法治療聲帶突肉芽腫的有效性,以及對(duì)患者咽喉部癥狀和體征的緩解情況。方法:選取2014-12-2016-06期間就診并符合納入條件的38例聲帶突肉芽腫患者,根據(jù)患者的治療意愿分為PPI聯(lián)合胃動(dòng)力藥物組(28例,其中10例為肉毒素注射失敗患者)及肉毒素注射治療組(20例)。用癥狀指數(shù)量表及體征評(píng)分量評(píng)估患者治療前后的癥狀及體征。結(jié)果:2組患者的年齡、性別、病程之間差異無(wú)統(tǒng)計(jì)學(xué)意義。PPI聯(lián)合胃動(dòng)力藥物治療的總有效率為96.43%;肉毒素注射組總有效率為45.00%。治療前2組患者反流癥狀指數(shù)量表及咽喉反流體征量表評(píng)分差異無(wú)統(tǒng)計(jì)學(xué)意義;肉芽腫完全消失、復(fù)發(fā)時(shí),2組患者反流癥狀指數(shù)量表及咽喉反流體征量表評(píng)分之間差異有統(tǒng)計(jì)學(xué)意義。結(jié)論:PPI聯(lián)合胃動(dòng)力藥物治療聲帶突肉芽腫的有效率及患者咽喉部癥狀和體征緩解率較肉毒素注射明顯要高。
[Abstract]:Objective: to compare the efficacy of two methods in the treatment of vocal cord granuloma by proton pump inhibitor (PPI) combined with gastric motility drug therapy and botulinum toxin injection under supporting laryngoscope. And the relief of symptoms and signs of pharynx and larynx. Methods: a total of 38 patients with vocal cord granuloma were selected from 2014-12 to 2016-06 and were divided into two groups: PPI combined with gastric motility drugs group (n = 28). There were 10 cases of botulinum toxin injection failure and 20 cases of botulinum toxin injection group. The symptoms and signs of patients before and after treatment were evaluated with symptom index scale and sign score. Results there was no significant difference in age, sex and course of disease between the two groups. The total effective rate of PPI combined with gastric motility drugs was 96.43 and the total effective rate of botulinum toxin injection group was 45.00. There was no significant difference in the scores of reflux symptom index scale and throat reflux sign scale between the two groups before treatment, and the granuloma disappeared completely. The scores of reflux symptom index scale and throat reflux sign scale were significantly different between the two groups at the time of recurrence. Conclusion the effective rate of the treatment of vocal cord granulomatosis and the relief rate of symptoms and signs of pharynx and larynx of the patients treated with the combination of 10% PPI and gastric motility drugs were significantly higher than that of the injection of botulinum toxin.
