【摘要】：目的研究干眼患者癥狀與淚膜脂質(zhì)層厚度(LLT)的相關(guān)關(guān)系。設(shè)計(jì)回顧性病例系列。研究對(duì)象隨機(jī)選取2015年7-10月就診于北京同仁醫(yī)院眼科門診的干眼患者139例,其中男性58例,女性81例,年齡18~77歲,平均(37.75±12.86)歲。方法所有入組患者均進(jìn)行眼表疾病指數(shù)(OSDI)問(wèn)卷(共包括12項(xiàng)癥狀)調(diào)查以及用Lipi View~汶眼表面光干涉儀測(cè)量LLT。根據(jù)干眼癥狀(OSDI問(wèn)卷評(píng)分)分為三組:輕度癥狀組35例(0~20分),中度癥狀組61例(21~45分),重度癥狀組43例(46~100分)。分析干眼癥狀與LLT的相關(guān)性以及不同程度干眼癥狀與LLT的關(guān)系。主要指標(biāo)OSDI問(wèn)卷評(píng)分、淚膜脂質(zhì)層厚度。結(jié)果 139例干眼患者OSDI評(píng)分與LLT呈顯著相關(guān)且為負(fù)相關(guān)(r=-0.256,P=0.002),隨著OSDI得分增加(癥狀的加重)脂質(zhì)層厚度變薄。OSDI問(wèn)卷中異物感、眼酸痛、看電視、看電腦眼部不適與LLT有明顯相關(guān)性(r=-0.206、-0.232、-0.190、-0.179,P均0.05),其他8項(xiàng)癥狀與LLT無(wú)相關(guān)性。嚴(yán)重程度不同的三個(gè)組別之間LLT存在差異,尤其是輕度癥狀組(64.89±16.40 nm)與重度癥狀組(54.65±14.48 nm)之間差異更明顯(F=10.092,P=0.027);輕度癥狀組中LLT≤60 nm者14例(40%),而重度組中LLT≤60 nm者30例(70%)(χ~2=6.953,P=0.008)。結(jié)論干眼癥狀與淚膜脂質(zhì)層厚度呈明顯負(fù)相關(guān)。干眼癥狀中的異物感、眼酸痛、看電視或電腦眼部不適等癥狀與淚膜脂質(zhì)層厚度相關(guān)性較顯著。淚膜脂質(zhì)層厚度低于60 nm者干眼癥狀嚴(yán)重。
[Abstract]:Objective to study the correlation between dry eye symptoms and tear film lipid layer thickness (LLTT). Design retrospective case series. Participants 139 dry eye patients (58 males and 81 females, 1877 years old with an average age of 37.75 鹵12.86) were selected randomly from July to October 2015 in the ophthalmological outpatient department of Tongren Hospital in Beijing. Methods all the patients were investigated with OSD I questionnaire (including 12 symptoms) and measured by Lipi View- Munn Surface Light Interferometer. According to the score of OSDI of xerophthalmia, it was divided into three groups: mild symptom group (35 cases), moderate symptom group (61 cases) and severe symptom group (43 cases). To analyze the correlation between xerophthalmia and LLT and the relationship between xerophthalmia and LLT. Main outcome measures OSDI questionnaire score, tear film lipid layer thickness. Results there was a significant correlation between OSDI score and LLT in 139 patients with dry eye and a negative correlation was found. With the increase of OSDI score, the thickness of lipid layer became thinner. There was a significant correlation between computer ocular discomfort and LLT. There was no correlation between the other 8 symptoms and LLT.
