【摘要】：目的評(píng)價(jià)中文版簡明兒童阻塞性睡眠呼吸暫停綜合征(OSAS)篩查量表(簡稱量表)在打鼾兒童中的信效度和篩查OSAS的能力。方法漢化量表,包括問題Q1~Q6。前瞻性收集2015年1月至2015年12月在上海交通大學(xué)醫(yī)學(xué)院附屬上海兒童醫(yī)學(xué)中心(我院)睡眠門診和耳鼻咽喉科門診就診的、以夜間睡眠打鼾為主訴的、年齡≥3歲的連續(xù)病例,在行PSG前由患兒家長回顧孩子近6個(gè)月的睡眠情況自行填寫量表,以多導(dǎo)睡眠監(jiān)測(PSG)作為診斷OSAS的金標(biāo)準(zhǔn)(阻塞性呼吸暫停低通氣指數(shù)(OAHI)每小時(shí)≥5次)�？疾炝勘淼男哦群托Ф�,以O(shè)AHI每小時(shí)1次、3次和5次界值分組考察量表篩查兒童OSAS的效能。結(jié)果符合本文納入、排除和剔除標(biāo)準(zhǔn)的425例夜間睡眠打鼾兒童進(jìn)入本文分析,以O(shè)AHI≥每小時(shí)5次為診斷標(biāo)準(zhǔn),82例(19%)被診斷為OSAS(OSAS組),余為非OSAS組。兩組性別、最低血氧飽和度和阻塞性呼吸暫停低通氣指數(shù)(AHI)差異有統(tǒng)計(jì)學(xué)意義。量表篩查OSAS的整體內(nèi)在信度克朗巴赫α系數(shù)0.785。對(duì)量表行探索性因子分析,抽樣適度度量為0.783,Bartlett's球形檢驗(yàn)P0.001,適合進(jìn)行因子分析,共提取2個(gè)因子(對(duì)各問題),載荷系數(shù)均0.5。以O(shè)AHI每小時(shí)≥5次、3次和1次作為OSAS組和非OSAS組的分組標(biāo)準(zhǔn)時(shí),最佳臨界值分別為2.32、2.32和1.99,敏感度均65%,特異度50%~60%。結(jié)論量表具有較好的信度和效度,可以快速有效地用于在夜間打鼾兒童中篩查中重度OSAS。
[Abstract]:Objective to evaluate the reliability and validity of simple Chinese version of OSAS scale for children with obstructive sleep apnea syndrome (OSAS) and its ability to screen for OSAS in snoring children. Methods Chinese scale, including Q1 Q6. From January 2015 to December 2015, consecutive cases of nocturnal sleep snoring, aged more than 3 years, were collected from Shanghai Children's Medical Center, affiliated to Shanghai Children's Medical Center, Shanghai Jiaotong University, from January 2015 to December 2015. Before PSG was performed, the parents of the children reviewed their children's sleep status in the past 6 months and filled out the scale by themselves.
