【摘要】：目的探討耳內(nèi)鏡、顳骨高分辨率CT、鼓室聲導(dǎo)抗及純音測(cè)聽等檢查對(duì)兒童分泌性中耳炎(secretory otitis media,SOM)的診斷價(jià)值。方法對(duì)207例(414耳)腺樣體肥大患兒分別行耳內(nèi)鏡、鼓室聲導(dǎo)抗、純音測(cè)聽及顳骨高分辨率CT檢查,以顳骨CT檢查結(jié)果作為分泌性中耳炎的診斷標(biāo)準(zhǔn),分別計(jì)算耳內(nèi)鏡、聲導(dǎo)抗及純音測(cè)聽診斷分泌性中耳炎的準(zhǔn)確度。結(jié)果 207例414耳中,CT診斷SOM(中耳積液)125耳(30.19%,125/414),其中116耳(92.80%,116/125)鼓膜穿刺證實(shí)有中耳積液;與顳骨CT結(jié)果比較,耳內(nèi)鏡(診斷中耳積液128耳,其中CT檢查陰性4耳)、鼓室聲導(dǎo)抗(B型導(dǎo)抗圖133耳,其中CT檢查陰性8耳)、純音測(cè)聽(0.5~4kHz氣骨導(dǎo)差≥15dB 81耳,其中CT檢查陰性2耳)三種檢查診斷SOM的敏感性分別為99.20%(124/125)、100%(125/125)、63.20%(79/125),特異性分別為98.62%(285/289)、97.23%(281/289)、99.31%(287/289),診斷符合率分別為98.79%(409/414)、98.07%(406/414)、88.41%(366/414)。耳內(nèi)鏡下見鼓室積液聯(lián)合B型鼓室導(dǎo)抗圖診斷SOM的符合率(99.52%)高于任一種單項(xiàng)檢查和其他任何兩種聯(lián)合檢查。結(jié)論以耳內(nèi)鏡下見鼓室積液聯(lián)合B型鼓室導(dǎo)抗圖診斷分泌性中耳炎準(zhǔn)確、安全。
[Abstract]:Objective to evaluate the diagnostic value of endoscopy, high resolution CTs of temporal bone, tympanic acoustic impedance and pure tone audiometry in secretory otitis mediaomus in children with secretory otitis media. Methods 207 children with adenoid hypertrophy were examined by endoscopy, tympanic acoustic conductance, pure tone audiometry and high resolution CT examination of temporal bone.
