【摘要】：目的比較鼓室內(nèi)注射與耳后注射甲基潑尼松龍治療難治性突發(fā)性耳聾的效果。方法選取溫州市中心醫(yī)院2014年3月至2015年12月確診并收治的62例難治性耳聾患者,按隨機(jī)數(shù)字表法分為觀察組和對(duì)照組,每組31例。所有患者根據(jù)病情持續(xù)使用微循環(huán)優(yōu)化、營養(yǎng)神經(jīng)等常規(guī)治療方案。在此基礎(chǔ)上,觀察組采取鼓室內(nèi)注射甲基潑尼松龍,對(duì)照組采取耳后注射甲基潑尼松龍,對(duì)比兩組療效、各頻率聽力改善情況及不良反應(yīng)。結(jié)果觀察組總有效率略高于對(duì)照組,但差異無統(tǒng)計(jì)學(xué)意義。兩組各頻率平均聽閾均有所改善,在0.5、1及3k Hz聽閾的改善幅度組間無明顯差異,而觀察組在4、8k Hz聽閾的改善幅度明顯優(yōu)于對(duì)照組。兩組不良反應(yīng)均輕微、可耐受,自行緩解。結(jié)論鼓室內(nèi)注射與耳后注射甲基潑尼松龍治療難治性突發(fā)性耳聾效果均較好,而耳后注射方案主要針對(duì)低、中頻聽力下降的患者。
[Abstract]:Objective to compare the effect of intratympanic injection and retroauricular injection of methylprednisolone in the treatment of refractory sudden deafness. Methods from March 2014 to December 2015, 62 patients with refractory deafness in Wenzhou Central Hospital were selected and divided into observation group (n = 31) and control group (n = 31). All patients continued to use microcirculation optimization, nutritional nerve and other routine treatment according to the condition. On this basis, the observation group received intratympanic injection of methylprednisolone and the control group received retroauricular injection of methylprednisolone. Results the total effective rate of the observation group was slightly higher than that of the control group, but the difference was not statistically significant. The average hearing threshold of each frequency in the two groups was improved, and there was no significant difference between the two groups in the improvement amplitude of 0.5kHz and 3kHz hearing threshold, but the improvement range of the observation group was obviously better than that of the control group at 4m 8kHz. The adverse reactions in both groups were mild, tolerable and self-remission. Conclusion both intratympanic injection and retroauricular injection of methylprednisolone are effective in the treatment of refractory sudden hearing loss.
