【摘要】：目的:通過(guò)對(duì)與凝血功能相關(guān)血液指標(biāo)的檢測(cè),探討阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患者凝血功能的變化及其臨床意義。方法:選擇我院2014年1月~2016年7月期間55例OSAHS男性患者為研究組,分為輕中度阻塞組、重度阻塞組和輕中度低血氧組、重度低血氧組。以55例聲帶小結(jié)患者為對(duì)照組,檢測(cè)研究組及對(duì)照組靜脈血凝血酶原時(shí)間(PT)、部分凝血活酶時(shí)間(APTT)、凝血酶時(shí)間(TT)、血漿纖維蛋白原(FIB)、血小板數(shù)量(PLT)、血小板平均體積(MPV)的水平、血小板壓積(PCT)、血小板分布寬度(PDW)。并在OSAHS組與對(duì)照組之間、不同阻塞程度和不同缺氧程度OSAHS患者之間進(jìn)行比較,對(duì)結(jié)果進(jìn)行分析,對(duì)相關(guān)問(wèn)題進(jìn)行討論。結(jié)果:研究組與對(duì)照組相比,研究組PLT高于對(duì)照組,分別是244.71±52.29×10~9/L和215.27±50.67×10~9/L,差異有統(tǒng)計(jì)學(xué)意義(P=0.003)。PCT高于對(duì)照組,分別為0.203±0.037%和0.184±0.035%,差異有統(tǒng)計(jì)學(xué)意義(P=0.005)。余檢測(cè)指標(biāo)無(wú)明顯差異(P0.05)。輕中度阻塞組與重度阻塞組相比,重度阻塞組RBC高于輕中度阻塞組,分別為5.31±0.69×1012/L和4.90±0.39×1012/L,差異有統(tǒng)計(jì)學(xué)意義(P=0.007).余各項(xiàng)指標(biāo)對(duì)比無(wú)明顯差異(P0.05)。輕中度底血氧組與重度底血氧組相比各項(xiàng)血生化指標(biāo)對(duì)比無(wú)明顯差異(P0.05)。結(jié)論:OSAHS患者存在血小板計(jì)數(shù)和血小板壓積的影響,但總體凝血相關(guān)臨床指標(biāo)未見(jiàn)異常。
[Abstract]:Objective: to investigate the changes of coagulation function in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and its clinical significance. Methods: from January 2014 to July 2016, 55 male patients with OSAHS in our hospital were divided into three groups: mild to moderate obstructive group, severe obstructive group, mild to moderate hypoxic group and severe hypoxic group. The levels of prothrombin time (PTT), partial thromboplastin time (APTTT), thrombin time (TTT), plasma fibrinogen (FIBN), platelet count (PLT) and mean platelet volume (MPV) were measured in 55 patients with vocal nodules. Platelet pressure-accumulation (PCT) and platelet distribution width (PDW). The results were compared between OSAHS group and control group, OSAHS patients with different degrees of obstruction and hypoxia. The results were analyzed and the related problems were discussed. Results: compared with the control group, the PLT in the study group was higher than that in the control group (244.71 鹵52.29 脳 10 ~ (9 / L) and 215.27 鹵50.67 脳 10 ~ (9 / L) / L respectively), the difference was statistically significant (0.203 鹵0.037% and 0.184 鹵0.035%, respectively) compared with the control group. There was no significant difference in the remaining detection indexes (P0.05). The RBC of severe occlusion group was higher than that of mild to moderate occlusion group (5.31 鹵0.69 脳 10 12 / L and 4.90 鹵0.39 脳 10 12 / L respectively) compared with that of severe occlusion group (P 0.007). There was no significant difference in other indexes (P 0.05). There was no significant difference in blood biochemical indexes between mild and moderate basal blood oxygen group and severe basal blood oxygen group (P 0.05). Conclusion the effect of platelet count and platelet hematocrit was found in patients with OSAHS, but there was no abnormality in the clinical parameters related to total coagulation.
