玻璃體切割聯(lián)合晶狀體置換治療中青年高度近視性黃斑劈裂的療效

【摘要】:目的探討玻璃體切割聯(lián)合晶狀體置換治療中青年高度近視黃斑劈裂的療效。方法將我院眼科2013年9月至2015年10月中青年高度近視黃斑劈裂患者60眼按手術(shù)方式不同隨機分為聯(lián)合手術(shù)組(30眼)和分期手術(shù)組(30眼),聯(lián)合手術(shù)組行玻璃體切割聯(lián)合晶狀體置換手術(shù),分期手術(shù)組單純行玻璃體切割手術(shù),術(shù)后根據(jù)晶狀體混濁程度擇期行白內(nèi)障手術(shù)。術(shù)后隨訪6~12個月,觀察分期手術(shù)組二次手術(shù)率,兩組術(shù)前術(shù)后最佳矯正視力(best corrected visual acuity,BCVA)、遠近視力,術(shù)后6個月黃斑區(qū)視網(wǎng)膜復(fù)位情況并行視覺質(zhì)量調(diào)查問卷。結(jié)果分期手術(shù)組術(shù)后晶狀體混濁加重27只眼(90%),因此行Ⅱ期白內(nèi)障手術(shù)21只眼(70%)。聯(lián)合手術(shù)組BCVA恢復(fù)、術(shù)后遠視力及術(shù)前術(shù)后遠視力差值明顯高于分期手術(shù)組(P0.05)。兩組術(shù)前、術(shù)后黃斑區(qū)視網(wǎng)膜情況及視網(wǎng)膜復(fù)位率無明顯差異(P0.05)。術(shù)后6個月視覺質(zhì)量綜合評分聯(lián)合手術(shù)組優(yōu)于分期手術(shù)組(t=-3.95,P=0.00),聯(lián)合手術(shù)組在遠視力、立體視覺、視疲勞項目上評分占優(yōu)勢,分期手術(shù)組在干眼、夜間眩光項目上評分高于聯(lián)合手術(shù)組(均P0.05)。結(jié)論玻璃體切割聯(lián)合晶狀體置換治療中青年高度近視黃斑劈裂有效避免二次手術(shù),改善視功能,是臨床上可行的手術(shù)方式。
[Abstract]:Objective to evaluate the effect of vitrectomy combined with lens replacement in the treatment of high myopia macular splitting in young and young people. Methods from September 2013 to October 2015, 60 eyes with high myopia macular splitting in our hospital were randomly divided into combined operation group (30 eyes) and staging group (30 eyes). Vitrectomy was performed in the combined operation group. Combined with lens replacement, The patients in the staging group underwent vitrectomy and cataract surgery according to the degree of lens opacity. After 6 ~ 12 months follow-up, the rate of secondary operation in the staging group was observed. The best corrected visual acuity and visual acuity were observed in the two groups. The retinal reattachment in macular area 6 months after operation and the visual quality questionnaire were performed in the two groups.
